血清抵抗素、鸢尾素对妊娠期糖尿病产后血糖转归的预测价值

摘　　要：目的探讨妊娠期糖尿病(GDM)患者血清抵抗素(Resistin)、鸢尾素(Irisin)表达情况及对GDM患者产后血糖转归预测价值。方法选取2019年1月至2020年6月在西南医科大学附属医院产检并住院分娩的GDM患者183例,根据患者产后8周血糖转归情况分为转归良好组(103例)和转归不良组(80例)。采用酶联免疫吸附法检测血清Resistin、Irisin水平,采用多因素logistic回归分析GDM患者产后血糖转归不良的影响因素,绘制ROC曲线分析Resistin、Irisin表达对GDM患者产后血糖转归不良的诊断价值。结果转归良好组GDM患者的孕前体重指数、糖尿病确诊的孕龄时期、甘油三酯(TG)、总胆固醇(TC)、胰岛素使用量与转归不良组比较,差异有统计学意义(P<0.05);转归不良组GDM组患者的血清Resistin水平高于转归良好组、Irisin表达水平低于转归良好组(P<0.05)。多因素logistic回归分析显示,确诊孕龄时期、TC、Resistin、Irisin是GDM患者产后血糖转归不良的独立影响因素(P<0.05)。血清Resistin、Irisin联合预测GDM患者产后血糖转归不良的敏感度为86.25%,特异度为88.35%,曲线下面积为0.924。结论GDM患者血清中Resistin、Irisin联合检测对GDM患者产后血糖转归不良具有较高诊断价值,可能作为预测GDM患者产后血糖转归不良潜在的标志物。Objective To explore the expression of Resistin and Irisin in serum of patients with gestational diabetes mellitus(GDM)and the value of predicting the outcome of postpartum blood glucose in patients with GDM.Methods A total of 183 patients with GDM who were examined and hospitalized for delivery in the Affiliated Hospital of Southwest Medical University from January 2019 to June 2020 were selected.They were divided into good outcome group(103 cases)and bad outcome group(80 cases)according to their blood glucose outcomes at 8 weeks after delivery.Enzyme linked immunosorbent assay was used to detect serum Resistin and Irisin levels,and multi-factor logistic regression was used to analyze the influencing factors of poor postpartum blood glucose outcome in GDM patients.ROC curve was plotted to analyze the diagnostic value of serum Resistin and Irisin expression in poor postpartum blood glucose outcome in GDM patients.Results The pre-pregnancy body mass index,gestational age of diabetes diagnosis,triglyceride(TG),total cholesterol(TC),and insulin usage of GDM patients in the good outcome group were compared with those in the poor outcome group,the differences were statistically significant(P<0.05).Serum Resistin level in GDM patients with poor outcome was higher than that in good outcome group,and the expression level of Irisin was lower than that in good outcome group(P<0.05).Multivariate logistic regression analysis showed that gestational age,TC,Resistin and Irisin were independent influencing factors of poor postpartum blood glucose outcome in GDM patients(P<0.05).The sensitivity,specificity and area under the curve of serum Resistin and Irisin combined in predicting poor postpartum blood glucose outcome in GDM patients were 86.25%,88.35%,and 0.924.Conclusion The combined detection of Resistin and Irisin in serum of GDM patients has high diagnostic value for poor postpartum blood glucose outcome in GDM patients,and may be used as a potential marker for predicting poor postpartum blood glucose outcome in GDM patien
