摘　　要：目的探讨改良Y形髂动脉分支支架在腹主动脉瘤腔内修复术中保留髂内动脉的方法及疗效。方法分析南京大学医学院附属鼓楼医院2018年9月至2020年8月21例使用改良Y形髂动脉分支支架行血管腔内修复术治疗的累及髂总动脉的腹主动脉瘤或夹层动脉瘤患者资料。患者术前经增强CT评估,术中结合造影情况由操作者制备Y形髂动脉分支支架行全腔内修复。结果患者手术均成功完成,手术中位时间220 min,共重建髂内动脉21根,使用覆膜支架23枚。术后平均随访时间为(8.9±3.4)个月,患者均未见动脉瘤腔增大,未见内漏及支架移位,无发生死亡。结论改良Y形髂动脉分支支架是一种有效的全腔内手术方法,方便快捷且适应证广,早中期疗效良好。Objective To explore the method and curative effect of the modified Y-shaped iliac artery branch stent in the endovascular repair of abdominal aortic aneurysm to preserve the internal iliac artery.Methods The clinical data of 21 patients with abdominal aortic aneurysm or dissecting aneurysm involving the common iliac artery treated with the modified Y-shaped iliac artery branch stent from September 2018 to August 2020 in the Nanjing Drum Tower Hospital were analyzed.Patients were evaluated by enhanced CT before operation,and operators prepared Y-shaped iliac artery branch stents for whole-cavity surgery according to intraoperative angiography during the operation.Results All patients were successfully completed the operations.The median operation time was 220 minutes.A total of 21 internal iliac arteries were reconstructed and 23 stent grafts were used.The average follow-up time was(8.9±3.4)months.No patients had any enlargement of the aneurysm cavity.No endoleaks,stent displacement or death occurred.Conclusion Modified Y-shaped iliac artery stent is an effective whole-cavity surgery method,which is convenient and fast,has a wide range of indications,and has good early and mid-term curative effects.
