摘  要:目的探讨长链非编码RNA NR_027032(AGAP2-AS1)、NR_047116(HIF1A-AS1)、NR_104181及环境因素与高血压病的关联,为高血压病的防治及表观遗传机制探讨提供理论基础。方法采用病例对照研究设计方法,通过面对面问卷调查及现场体检,对福州市某医院高血压病患者(病例组,439例)和性别、年龄均衡可比的非高血压病患者(对照组,439例)进行调查,并采用实时荧光定量PCR测定62对病例组及对照组血样的NR_027032(AGAP2-AS1)、NR_047116(HIF1A-AS1)及NR_104181的表达量。结果多因素Logistic回归分析结果显示:行为及心理等环境因素中,清淡饮食(OR=0.583,95%CI:0.421~0.805)是高血压病患病的保护因素;被动吸烟(OR=1.489,95%CI:1.004~2.207)、腹型肥胖(OR=1.769,95%CI:1.210~2.586)、超重[24.0≤体质量指数(BMI,kg/m^(2))<28.0](OR=1.527,95%CI:1.096~2.127)、肥胖(BMI≥28.0)(OR=2.042,95%CI:1.104~3.780)、轻度抑郁(OR=1.435,95%CI:1.010~2.038)、中重度抑郁(OR=1.783,95%CI:1.181~2.693)、A型性格(OR=1.740,95%CI:1.208~2.506)是其危险因素。非参数检验结果显示:与对照组比较,病例组的lncRNA NR_027032(AGAP2-AS1)、NR_047116(HIF1A-AS1)及NR_104181的表达量较低(P<0.05)。在控制年龄、性别、学历、婚姻、BMI、抑郁程度等因素后发现,NR_047116(OR=0.631,95%CI:0.408~0.978)和NR_104181(OR=0.611,95%CI:0.378~0.988)低表达可能是高血压病的危险因素。结论环境和表观遗传因素与高血压病的发生发展存在关联,长链非编码RNA NR_047116(HIF1A-AS1)和NR_104181低表达可能与高血压病的患病风险增加存在关联。Objective To explore the main environmental risk factors of hypertension and understand the role of long-chain non coding RNAs NR_027032(AGAP2-AS1),NR_047116(HIF1A-AS1),NR_104181 in the development of hypertension.Methods Using the method of case-control study,we conducted questionnaire surveys,physical examinations and laboratory tests of hypertension cases and healthy people in a hospital of Fuzhou from March 2015 to December 2017.Real-time fluorescent quantitative PCR was used to detect the expression of long non coding RNA(AGAP2-AS1,HIF1A-AS1 and NR_104181)in case group and control group.Multivariate logistic regression analysis was used to explore whether environmental factors and AGAP2-AS1,HIF1A-AS1,NR_104181 were associated with hypertension.Results Logistic regression analysis of hypertension environmental factors from 439 cases and 439 controls showed that:passive smoking(OR=1.489,95%CI:1.004-2.207),abdominal obesity(OR=1.769,95%CI:1.210-2.586),overweight(24.0≤BMI<28.0 kg/m^(2))(OR=1.527,95%CI:1.096-2.127),obesity(BMI≥28.0 kg/m^(2))(OR=2.042,95%CI:1.104-3.780),mild depression(OR=1.435,95%CI:1.010-2.038),moderate to severe depression(OR=1.783,95%CI:1.181-2.693),type A personality(OR=1.740,95%CI:1.208-2.506)and a light diet(OR=0.583,95%CI:0.421-0.805)were factors for hypertension.The expression levels of AGAP2-AS1,HIF1A-AS1 and NR_104181 in peripheral blood leukocytes of the case group were lower than those of the control group(P<0.05),which may be associated with hypertension.Logistic regression analysis of environmental factors and genetic factors showed that after controlling for factors such as age,gender,abdominal obesity,drinking status,tea drinking status,passive smoking,light diet,depression degree,HIF1A-AS1(OR=0.631,95%CI:0.408-0.978)and NR_104181(OR=0.611,95%CI:0.378-0.988)low expression could increase the risk of hypertension.Conclusion Environmental and epigenetic factors are associated with the development of hypertension.The low expression of long noncoding RNA HIF1A-AS1 and NR_104181 may
