超声造影在腹主动脉瘤腔内修复术后内漏检测中的应用

摘　　要：目的探讨超声造影(CEUS)在腔内主动脉修复(EVAR)术后随访中的应用价值。方法回顾性研究2019年7月1日-2020年11月1日收治并成功实施EVAR术的肾下型腹主动脉瘤患者共62例,术后1个月内分别行CT血管造影(CTA)及CEUS检查,两种检查的相隔时间<1周。分别记录内漏发生情况、瘤体直径和检查时长。结果CTA判断总的内漏发生率为22.58%,CEUS为25.81%,二者对1型/3型内漏的判断一致,1型2例,3型1例;而对2型内漏,CTA诊断了11例,CEUS诊断了13例,CEUS比CTA多发现的2例均为EVAR术中使用瘤腔弹簧圈栓塞患者。CTA测量瘤体的直径为(54.2±9.2)mm,CEUS为(53.4±11.1)mm,二者差别无统计学意义(P>0.05)。结论CEUS应用于腹主动脉瘤EVAR术后随访是安全有效的。Objective To investigate the value of CEUS in the follow-up of patients treated with EVAR.Methods Retrospective analysis of the patients admitted between July 2019 and November 2020,who were diagnosed with infra-renal abdominal aortic aneurysm and treated with EVAR,was conducted via a prospective database.Sixty-two patients were studied with both CTA and CEUS with interval time less than a week.The type of endoleak,time of test,and diameter of aneurysm were recorded.Results Overall occurrence rate of endoleak was 22.58%with CTA and 25.81%with CEUS.Both were accordant towards type 1/3 endoleak.CEUS found two more cases of type 2 endoleak than CTA,which was attributed to the artefacts produced by coils when CTA was performed.No difference was found in the measurement of aneurysm diameter.Conclusion CEUS is a safe,effective tool for post-EVAR follow-up with excellent repeatability and patient satisfaction.
