益气通络颗粒联合银杏达莫注射液治疗缺血性脑血管病伴认知功能障碍临床研究

摘　　要：目的探讨益气通络颗粒联合银杏达莫注射液治疗缺血性脑血管病伴认知功能障碍的疗效,以及对患者前列环素I2(PGI2)和血栓素A2(TXA2)水平的影响。方法选取医院2018年5月至2020年5月收治的患者105例,按随机抽样法分为研究组(54例)和对照组(51例)。两组患者均予银杏达莫注射液治疗,观察组患者在对照组基础上加用益气通络颗粒,均治疗4周。结果研究组总有效率为88.89%,显著高于对照组的72.55%(P<0.05)。与对照组比较,治疗后研究组患者的简易精神状态评价量表、蒙特利尔认知评估量表评分均显著升高,神经功能缺损评分量表评分显著降低(P<0.05);PGI2水平显著升高,TXA2水平显著降低(P<0.05);患者的脑血流灌注指标平均通过时间显著缩短,脑血容量、脑血流量均显著升高(P<0.05)。研究组和对照组不良反应发生率相当(16.67%比11.76%,P>0.05)。结论益气通络颗粒联合银杏达莫注射液治疗缺血性脑血管病伴认知功能障碍疗效良好,能改善患者的认知功能,调节机体PGI2和TXA2水平,改善脑血流灌注,且治疗安全性良好。Objective To investigate the efficacy of Yiqi Tongluo Granules combined with Ginkgo Leaf Extract and Dipyridamole Injection in the treatment of ischemic cerebrovascular disease and cognitive impairment and the effect on the levels of prostacyclin I2(PGI2)and thromboxane A2(TXA2).Methods A total of 105 patients with ischemic cerebrovascular disease and cognitive impairment admitted to our hospital from May 2018 to May 2020 were selected and randomly divided into the study group(54 cases)and the control group(51 cases).The patients in the observation group were treated with Ginkgo Leaf Extract and Dipyridamole Injection,on this basis,the patients in the observation group were treated with Yiqi Tongluo Granules.Both groups were treated for four weeks.Results The total effective rate of the study group was 88.89%,which was significantly higher than 72.55%of the control group(P<0.05).The scores of the Mini-Mental State Examination(MMSE)scale and the Montreal Cognitive Assessment(MoCA)scale in the study group were significantly higher than those in the control group,while the score of the National Insitute of Health Stroke Scale(NIHSS)in the study group was significantly lower than that in the control group(P<0.05).The level of PGI2 in the study group was significantly higher than that in the control group,while the level of TXA2 in the study group was significantly lower than that in the control group(P<0.05).The mean transit time(MTT)of cerebral blood flow perfusion index in the study group was significantly shorter than that in the control group,while the cerebral blood volume(CBV)and cerebral blood flow(CBF)in the study group were significantly higher than those in the control group(P<0.05).The incidence of adverse reactions in the study group was similar to that in the control group(16.67%vs.11.76%,P>0.05).Conclusion Yiqi Tongluo Granules combined with Ginkgo Leaf Extract and Dipyridamole Injection are effective and safe in the treatment of ischemic cerebrovascular disease and cognitive impairment,which can improv
