血府逐瘀汤联合穴位敷贴治疗心血瘀阻型心绞痛疗效观察

摘　　要：目的探讨血府逐瘀汤联合穴位敷贴治疗心血瘀阻型心绞痛的临床疗效,以及对患者血液流变学指标的影响。方法选取河北省保定市第二中心医院2019年3月至2021年3月收治的心血瘀阻型心绞痛患者105例,按随机抽签法分为观察组(53例)和对照组(52例)。对照组给予常规西药治疗,观察组在对照组基础上增加血府逐瘀汤联合穴位敷贴治疗。两组患者均以4周为1个疗程,连续治疗3个疗程。结果观察组总有效率为90.57%,显著高于对照组的73.08%(P<0.05)。治疗后,观察组左室射血分数、每搏输出量均显著高于对照组,左室舒张末期内径、QT间期离散度均显著低于对照组(P<0.05);观察组全血黏度、血浆黏度、红细胞变形指数、纤维蛋白原水平均显著低于对照组(P<0.05);观察组患者的血管内皮素1、同型半胱氨酸、心肌肌钙蛋白T水平均显著低于对照组(P<0.05);观察组与对照组的不良反应发生率相当(15.09%比7.69%,P>0.05)。结论血府逐瘀汤联合穴位敷贴治疗心血瘀阻型心绞痛的临床疗效良好,可改善患者的心功能和血液流变学指标,降低血脂代谢指标水平,且治疗安全性良好。Objective To investigate the clinical efficacy of Xuefu Zhuyu Decoction combined with acupoint application in the treatment of angina pectoris of heart blood stasis syndrome and its effect on hemorheological indexes.Methods A total of 105 patients with angina pectoris of heart blood stasis syndrome admitted to the Baoding Second Central Hospital from March 2019 to March 2021 were selected and divided into the observation group(53 cases)and the control group(52 cases)according to the random lottery method.The patients in the control group were given conventional Western medicine treatment,on this basis,the patients in the observation group were given Xuefu Zhuyu Decoction and acupoint application.Both groups were treated for three consecutive courses with four weeks as a course of treatment.Results The total effective rate of the observation group was 90.57%,which was significantly higher than 73.08%of the control group(P<0.05).After treatment,the left ventricular ejection fraction(LVEF)and stroke volume(SV)in the observation group were significantly higher than those in the control group,while the left ventricular end-diastolic diameter(LVEDd)and QT interval dispersion(QTd)in the observation group were significantly lower than those in the control group(P<0.05).The whole blood viscosity,plasma viscosity,erythrocyte deformation indexand fibrinogen level in the observation group were significantly lower than those in the control group(P<0.05).The levels of endothelin 1(ET-1),homocysteine(HCY)and cardiac troponin T(cTnT)in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(15.09%vs.7.69%,P>0.05).Conclusion Xuefu Zhuyu Decoction combined with acupoint application is effective and safe in the treatment of angina pectoris of heart blood stasis syndrome.It can improve the cardiac function and hemorheological indexes of patients,and reduce the levels of blood lipid metabolism.
