摘　　要：目的探讨临床药师在1例吗啡缓释片致老年晚期肿瘤患者蓄积中毒药学监护中的作用。方法临床药师查阅文献及药品说明书,总结阿片类药物的使用风险、中毒解救措施,以及老年肿瘤患者镇痛药物的选择方案,分析吗啡缓释片致该例患者蓄积中毒的原因,提出解救措施,并协助临床医师调整用药方案。结果经临床药师分析,吗啡蓄积中毒可能由患者肾功能不全导致,建议临床医师调整镇痛方案及纳洛酮治疗方案。临床医师接受建议,患者调整镇痛方案后的中毒症状逐渐好转。结论临床药师结合药学专业特点,开展用药分析,优化药物治疗方案,可确保患者用药安全、有效。Objective To investigate the role of clinical pharmacists in the pharmaceutical care of an elderly cancer patient with accumulated poisoning caused by Morphine Sustained-Release Tablets.Methods The clinical pharmacist searched the literature and drug manuals,summarized the use risk of opioids,the rescue measures of poisoning,and the choice of analgesic drugs for elderly cancer patients,analyzed the causes of the cumulative poisoning caused by Morphine Sustained-Release Tablets,put forward rescue measures,and assisted clinicians to adjust the medication plan.Results The clinical pharmacist analyzed that the patient may be poisoned by accumulation of morphine due to renal insufficiency,and suggested that the clinician adjust the analgesia plan and the naloxone treatment plan.The clinician accepted the suggestion,and the patient's symptoms of poisoning gradually improved.Conclusion The clinical pharmacist combines the characteristics of the pharmacy to conduct drug analysis and optimize the treatment plan,which can ensure the safety and effectiveness of patient's medication.
