镍钛形状记忆合金缝线两线法缝合与传统缝合在甲皱襞成形术中的对比研究

摘　　要：目的比较传统缝合与镍钛形状记忆合金缝线两线法缝合在手术治疗嵌甲症方面的疗效。方法选择2018年1月至2019年6月在我院就诊的Ⅱ期、Ⅲ期嵌甲症患者46例60趾,随机分为研究组23例30趾和对照组23例30趾,均采用改良Winograd术式治疗。研究组采用镍钛形状记忆合金缝线创新缝合成形甲皱襞,对照组采用传统间断缝合方法缝合。比较两组切口愈合情况、渗出情况、拆线时间、近期甲外形、复发情况。结果对照组术后(14.30±1.06)d拆除缝线,研究组(9.67±1.42)d拆除缝线,研究组23例共30趾侧嵌甲甲级愈合率达53.3%。所有患者应用VAS评分,研究组拆线时痛苦明显减轻,对甲皱襞术后外观满意度更高。结论应用镍钛形状记忆合金缝线两线法缝合治疗嵌甲症缩短了手术至拆线的时间,减轻了拆线时患者的痛苦,甲皱襞成形效果更好,更有利于嵌甲症术后的病情恢复。Objective To compare clinical effect of bi-thread suture with nitinol shape memory alloy suture and traditional suture in the surgical treatment of ingrown toenail.Methods 46 patients with stageⅡandⅢingrown toenails treated in our hospital from January 2018 to June 2019 were randomly divided into the study group and the control group,with 23 patients(30 toes)in each.They were all treated with modified Winograd operation.For the nailfold formation,the study group used nitinol shape memory alloy bi-thread suture,and the control group used traditional intermittent suture.The wound healing stage,exudation rate,suture removal time,recent nail appearance and recurrence rate were compared between the two groups.Results Post-operative stripping time was(14.30±1.06)days and(9.67±1.42)days of the control group and the study group(P<0.05).In the study group,the grade A healing rate was 53.3%in all the 23 cases with 30 toe sides.The post-operative VAS score significantly reduced,and the satisfaction with the appearance after nailfold operation was higher in the study group.Conclusion The application of the bi-thread suture with nitinol shape memory alloy can shorten the waiting period of postoperative suture removal,ease the pain of thread removal,and achieve more accurate nail fold formation,which is more conducive to the recovery of the condition after ingrown nail surgery.
