摘　　要：目的探讨外周血辅助性T[淋巴]细胞9(Th9)细胞因子水平与过敏性鼻炎患者疾病严重程度的相关性。方法选择100例过敏性鼻炎患者,按照病情严重程度分为轻度组36例、中度组49例、重度组15例。所有患者均使用流式细胞仪检测外周血Th9细胞比例,采用实时荧光定量qPCR法检测血浆转录因子PU.1 mRNA、干扰素调节因子4(IRF4)mRNA相对表达水平,采用酶联免疫吸附实验法检测血清白细胞介素(IL)-9、IL-4、转化生长因子β1(TGF-β1)水平,使用多因素Logistic回归分析Th9细胞因子水平与过敏性鼻炎患者疾病严重程度的相关性。结果轻度组、中度组、重度组外周血Th9细胞比例、IL-9水平、IL-4水平、TGF-β1水平、PU.1 mRNA及IRF4 mRNA的相对表达水平依次升高(均P<0.05)。血浆PU.1 mRNA、IRF4 mRNA相对表达水平及血清IL-9、IL-4、TGF-β1水平是过敏性鼻炎严重程度的影响因素(P<0.05)。结论随着过敏性鼻炎病情严重程度的发展,患者外周血Th9细胞比例和Th9细胞因子(PU.1 mRNA与IRF4 mRNA、IL-9、IL-4、TGF-β1)水平上升,且Th9细胞因子水平是过敏性鼻炎严重程度的影响因素。Objective To explore the correlation between peripheral blood levels of T helper[lymphocyte]9(Th9)cytokines and disease severity in patients with allergic rhinitis.Methods A total of 100 patients with allergic rhinitis were selected and classified into mild(n=36),moderate(n=49),and severe(n=15)groups according to disease severity.The proportion of peripheral blood Th9 cells was determined in all patients by flow cytometry,the relative expression levels of plasma transcription factor PU.1 mRNA and interferon regulatory factor 4(IRF4)mRNA were determined by real-time fluorescent quantitation PCR,and serum levels of interleukin(IL)-9,IL-4 and transforming growth factorβ1(TGF-β1)were measured by enzyme-linked immunosorbent assay,and multivariate Logistic regression analysis was performed on the correlation between Th9 cytokines and disease severity in allergic rhinitis patients.Results The proportion of peripheral blood Th9 cells,IL-9 level,IL-4 level,TGF-β1 level,and relative expression levels of PU.1 mRNA and IRF4 mRNA increased in the sequence of the mild,moderate and severe groups(all P<0.05).The relative expression levels of plasma PU.1 mRNA and IRF4 mRNA,as well as serum IL-9,IL-4 and TGF-β1 levels,were the influencing factors of the severity of allergic rhinitis(P<0.05).Conclusion Patients′proportion of peripheral blood Th9 cells,and levels of Th9 cytokines(PU.1 mRNA,IRF4 mRNA,IL-9,IL-4,and TGF-β1)increase with the progression of the severity of allergic rhinitis,moreover,and the levels of Th9 cytokines are the factors influencing the severity of allergic rhinitis.
