摘 要:目的系统评价睾酮替代疗法(TRT)对迟发性性腺功能减退患者下尿路症状、前列腺体积及前列腺特异性抗原水平的影响。方法通过计算机检索PubMed、Embase、Cochrane library、CNKI、万方、维普等数据库,筛选建库至2020年8月符合要求的随机对照试验(RCT)。由2名评价员按照纳入与排除标准选择文献、提取资料和评价质量后,采用RevMan 5.3软件进行荟萃分析。结果共纳入19篇研究,2479例患者。Meta结果显示短期睾酮替代疗法治疗增加前列腺体积(MD=2.48,95%CI:1.32~3.65,P<0.05),但不会增加中、长期前列腺体积;TRT在国际前列腺评分、最大尿流率、前列腺特异性抗原方面与对照组(安慰剂组)比较差异均无统计学意义(P>0.05)。结论TRT可能会增加前列腺体积,但对国际前列腺评分、最大尿流率及前列腺特异性抗原水平无明显影响。Objective To evaluate the effects of testosterone replacement therapy(TRT)on the lower urinary tract symptoms(LUTS),prostate volume(PV)and prostate-specific antigen(PSA)in late-onset hypogonadism(LOH)patients.Methods Controlled clinical trials about effects of TRT on LOH patients were searched in PubMed,Embase,Medline,Cochrane library,CNKI,Wangfan data and VIP database from inception to Aug.2020.Two independent reviewers screened the studies according to the inclusion and exclusion criteria,extracted the data,and evaluated the quality.Revman 5.3 was used for Meta-analysis.Results A total of 19 studies and 2479 patients were included.Meta-analysis showed that short-term TRT increased PV(MD=2.48,95%CI:1.32-3.65,P<0.05),but did not increase mid-term and long-term PV;TRT had no significant effects on International Prostate Symptoms Score(IPSS),maximum flow rate(Q max)and PSA(P>0.05).Conclusion Short-term TRT may increase prostate volume,but has no significant effect on IPSS,Q max and PSA level.
