摘 要:目的:观察脐带停止搏动后断脐在早产儿中的应用效果。方法:选取150例早产儿为研究对象,按照随机数字表法分为观察组与对照组各75例。对照组于早产儿娩出后10 s内断脐,观察组于早产儿娩出脐带停止搏动后断脐,比较两组早产儿出生后血红蛋白水平、经皮胆红素水平、贫血发生率和病理性黄疸发生率。结果:出生第3天、第7天,观察组血红蛋白水平均高于对照组,差异有统计学意义(P<0.05);两组经皮胆红素水平比较,差异均无统计学意义(P>0.05);观察组贫血发生率明显低于对照组,差异有统计学意义(P<0.05);两组病理性黄疸发生率比较,差异无统计学意义(P>0.05)。结论:早产儿娩出脐带停止搏动后断脐可提高早产儿出生后血红蛋白水平,以及降低贫血发生率,效果优于早产儿娩出后10 s内断脐。Objective:To observe application effects of omphalotomy after stopping the pulsation of the umbilical cord in premature infants.Methods:150 premature infants were selected as the research objects,and were divided into observation group and control group according to the random number table method,75 cases in each group.The control group was given omphalotomy within 10s after delivery,while the observation group was given omphalotomy after stopping the pulsation of the umbilical cord.The hemoglobin level,the skin bilirubin level,the incidence of anemia and the incidence of pathological jaundice were compared between the two groups after birth.Results:On the 3rd and 7th day of birth,the hemoglobin level of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the transcutaneous bilirubin level between the two groups(P>0.05).The incidence of anemia in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of pathological jaundice between the two groups(P>0.05).Conclusions:The omphalotomy after stopping the pulsation of the umbilical cord in the premature infants can increase the hemoglobin level after birth and reduce the incidence of anemia.Moreover,it is superior to omphalotomy within 10s after delivery.
