全凭静脉麻醉下术中阴茎勃起处理新思路

摘　　要：目的评估全凭静脉麻醉(TIVA)下行经尿道内窥镜手术术中阴茎勃起(IPE)的发生率以及发生后改变麻醉维持状态对其影响。方法回願性分析2018年1月至2019年12月行经尿道内窥镜手术且符合纳入标准的患者临床资料,包括悲者年龄、诊断、术前合并症、ASA分级、手术方式、手术时长,及发生IPE与否、IPE发生时机、处理方式及恢复时间等。结果共纳入1274例患者,其中15例患者发生IPE,发生率为1.2%。IPE患者中,常规处理(即暂停手术操作后等待)8例,平均年龄(57.0±17.6)岁,平均恢复时间(44.5±12.5)min,平均手术时间(168.1±24.6)min;改进处理(即暂停手术操作并中止麻醉维持后等待)7例,平均年龄(56.2±13.8)岁,平均恢复时间.(11.0±2.4)min,平均手术时间(89.3±30.7)min。结论TIVA下行经尿道内窥镜手术术中阴茎勃起发生率为1.2%,发生时改变麻醉维持状态是一种无创、省时、有效的IPE处理方法,可能为临床处理提供新思路。Objective To evaluate the incidence of penile erection during transurethral endoscopie surgery under total intravenous anesthesia and the efete of changing anesthesia maintenance starus afier it occur.Methods The clinical data ot paticnts who underwent transurechral endoscopic surgery in our hospital from January 2018 to December 2019 and met the inclusion criteria of this study were rtrospectively analyzed,including each patient's age,diagnosis,preoperative comorbidities.ASA clasification。operation method,operation duration.whether IPE occurs。timing of IPE occurrence。treatment methods and recovery time.Results A total of 1274 eligible patients were enrolled,of which 15 patients developed IPE,with an incidence rate of 1.2%Among IPE patients,8 were treated routinely(suspending ay operation fllowed by waiting),the average age(standard deviation)was 57.0(17.6)year,the avenge recovery time(standard deviation)was 44.5(12.5)minutes,and the average operation time(standard deviation)was 168.1(24.6)mimutes;7 cases in the improved treatment group(suspending any operarion and stopping anesthetic maintenance fllowed by waiting)7 cases,the average age(standard deviation)56.2(13.8)years,the average recovery time(standard deviation)11.0(2.4)minutes,the average openation Duration(standard deviation)89.3(30.7)minutes.Conclusions Our study shows that the incidence of penile erection during transurethral endoscopic surgery under total intravenous anesthesia is I.2%.Changing the anesthesia maintenance state when it occurs is a non-invasive.time-saving and ffective IPE treatment method,which may provide a new idea for clinical treatment.
