摘　　要：【目的】探讨狼疮性肾炎(LN)患者miR-127-3p、miR-129-5p分子的表达情况及与微观血瘀证和相关临床指标的相关性。【方法】收集符合微观血瘀证及微观非血瘀证的LN患者各30例,同时选取15例健康志愿者作为健康对照组。应用逆转录聚合酶链反应(RT-PCR)检测所有受试者血浆miR-127-3p、miR-129-5p分子表达量,比较2组LN患者肾功能[血尿素氮(BUN)、血尿酸(UA)、血肌酐(Cr)、血清胱抑素(Cys-C)]和肾病理指数[活动性指数(AI)、慢性化指数(CI)]的差异,分析miR-127-3p、miR-129-5p分子与上述临床指标的相关性。【结果】(1)微观血瘀证组患者miR-127-3p、miR-129-5p及Cr、BUN、UA、Cys-C、AI、CI水平均明显高于非血瘀证组,差异均有统计学意义(P<0.05)。微观血瘀证组miR-127-3p的表达量明显高于健康对照组(P<0.05),而2组LN患者miR-129-5p表达与健康对照组比较,差异均无统计学意义(P>0.05)。(2)LN患者miR-127-3p表达量与血Cr、BUN、UA、Cys-C、AI水平具有正相关性(P<0.05),miR-129-5p表达量与血Cr、BUN水平具有正相关性(P<0.05)。【结论】LN合并微观血瘀证患者肾功能更差,病理活动、慢性化程度均更高;MiR-127-3p、miR-129-5p分子与微观血瘀证具有一定相关性,有可能作为潜在的生物标志物以指导辨证。Objective To investigate the expression of miR-127-3 p and miR-129-5 p in patients with lupus nephritis(LN),and to study their correlation with micro-blood stasis syndrome and related clinical indicators.Methods The 30 cases of LN patients with micro-blood stasis syndrome and 30 LN patients without blood stasis syndrome were enrolled into the study. Meanwhile,15 healthy volunteers served as the healthy control group. The expression levels of mi R-127-3 p and miR-129-5 p in the plasma of all studying subjects were detected by reverse transcription polymerase chain reaction(RT-PCR). The renal function indicators of blood urea nitrogen(BUN),blood uric acid(UA),serum creatinine(Cr),and serum cystatin C(Cys-C),and the renal pathological indexes of activity index(AI)and chronic index(CI)in the two groups of LN patients were compared. And finally,the correlation of miR-127-3 p and miR-129-5 p expression levels with the renal function indicators and the pathological indexes was analyzed. Results(1)The levels of miR-127-3 p,miR-129-5 p,Cr,BUN,UA,Cys-C,AI,and CI in the LN patients with micro-blood stasis syndrome were significantly higher than those in the LN patients with non-blood stasis syndrome,and the differences were statistically significant(P< 0.05). The expression level of miR-127-3 p in micro-blood stasis syndrome LN group was obviously higher than those in the healthy control group(P< 0.05),but there was no significant difference in miR-129-5 p expression level between the two LN patients groups and the healthy control group(P > 0.05).(2)The expression level of miR-127-3 p in LN patients was positively correlated with the levels of blood Cr,BUN,UA,Cys-c and AI(P< 0.05),and the expression level of miR-129-5 p was positively correlated with the blood levels of Cr and BUN(P<0.05).Conclusion LN patients with micro-blood stasis syndrome present worse renal function and higher degree in pathological activity and chronicity.
