滋肾平眩汤联合倍他司汀治疗PCIV的疗效

摘　　要：目的探讨滋肾平眩汤联合倍他司汀治疗后循环缺血性眩晕(PCIV)肾阴不足证患者的效果及其对脑动脉血流、血液流变学的影响。方法根据随机数字表法将109例肾阴不足证PCIV患者分组,对照组54例予以倍他司汀治疗,观察组55例在对照组基础上增加滋肾平眩汤治疗。观察两组临床疗效、中医证候积分、脑动脉血流速度、血液流变学及不良反应发生情况。结果治疗后,观察组疗效优于对照组(P<0.05);观察组中医证候积分较对照组低(P<0.05);观察组全血黏度(WBV)、全血低切黏度、全血高切黏度、血浆黏度(PV)及纤维蛋白原(PAR)水平均较对照组低(P<0.05);观察组基底动脉(BA)和左、右侧椎动脉(LVA、RVA)血流速度较对照组快(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论滋肾平眩汤联合倍他司汀可通过调节PCIV(肾阴不足证)患者血液流变学,改善脑动脉血流速度,缓解临床症状,提高治疗效果。Objective To explore the effect of Zishen Pingxuan decoction combined with becastastine in the treat ment of posterior circulation ischemic vertigo(PCIV)and its effect on cerebral artery blood flow and hemorheology.Methods 109 cases of PCIV with renal Yin deficiency syndrome were randomly divided into two groups.54 cases in the control group was treated with betastastine,and 55 cases in the observation group was treated with ZishenPingxuan decoc tion on the basis of the control group.The clinical efficacy,TCM syndrome score,cerebral artery blood flow velocity,hemorheology and the incidence of adverse reactions of the two groups were observed.Results After treatment,the cura tive effect of the observation group was better than that in the control group(P<0.05).The TCM syndrome score of the ob servation group was lower than that of the control group(P<0.05).The whole blood viscosity(WBV),whole blood low cut viscosity,whole blood high-cut viscosity,plasma viscosity(PV)and fibrinogen(PAR)levels in the observation group were lower than those in control group(P<0.05).The blood flow velocity of basilar artery(BA),left vertebral artery(LVA)and right vertebral artery(RVA)in the observation group was faster than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Zishen Pingxuan decoction combined with Betastastine can improve cerebral artery blood flow velocity,alleviate clinical symp toms and improve therapeutic effect by regulating hemorheology in PCIV patients with kidney Yin deficiency type.
