摘  要:目的观察改良型配对血浆滤过吸附(coupled plasma filtration adsorption,CPFA)模式治疗血小板减少合并多器官功能障碍综合征危重患者的效果。方法选取2017年1月至2019年12月我院重症监护病房收治的血小板减少合并多器官功能障碍综合征危重患者140例,按照数字表法随机分为对照组和观察组,每组各70例。对照组应用常规CPFA模式,观察组应用改良型CPFA模式(血液灌流联合连续性血浆滤过治疗),比较应用两种模式治疗前、后患者红细胞血小板、尿素氮、肌酐水平的变化。结果(1)治疗后两组患者的红细胞计数、血小板计数均较治疗前有所下降,对照组红细胞计数、血小板计数治疗前、后差异具有统计学意义(P<0.05),观察组红细胞计数、血小板计数治疗前、后差异无统计学意义(P>0.05)。(2)治疗后两组患者的尿素氮、肌酐均较治疗前明显下降,差异具有统计学意义(P<0.05)。结论常规CPFA模式和改良型CPFA模式治疗血小板减少合并多器官功能障碍综合征患者的临床疗效显著,无并发症发生。改良型CPFA模式可减少对患者血液有效成分的破坏,临床应用更加安全。Objective To observe the clinical effects of modified coupled plasma filtration adsorption(CPFA)model vs the conventional CPFA model in critically ill patients with thrombocytopenia combined with multiple organ dysfunction syndrome.Methods From January 2017 to December 2019,140 critically ill patients with thrombocytopenia combined with multiple organ dysfunction syndrome in the intensive care unit of our hospital were selected and randomly divided into the control group and the observation group according to the random number table method,with 70 cases in each group.The control group was treated with conventional CPFA model,and the observation group was treated with modified CPFA model.The changes of erythrocyte,platelet,urea nitrogen and creatinine levels were compared before and after the treatment.Results The red blood cells and platelets in the two groups were decreased after the treatment,and the differences in the control group were statistically significant(P<0.05),while the differences in the observation group were not statistically significant(P>0.05).After the treatment,urea nitrogen and creatinine in the two groups were significantly lower than before,and the differences were statistically significant(P<0.05).Conclusion The clinical efficacy of conventional CPFA mode and modified CPFA mode in the treatment of patients with thrombocytopenia complicated with multiple organ dysfunction syndrome was significant without complications.The modified CPFA model reduced the damage to blood active components and was safer to apply.
