中药封包联合手法治疗对半月板损伤的疗效观察

摘　　要：目的探讨中药封包联合手法治疗对半月板损伤患者的临床疗效及对膝关节功能的影响。方法选取2019年1月至2020年12月在四川省骨科医院治疗的102例半月板损伤患者为研究对象,按照随机数字表法分为试验组和对照组,每组51例。对照组给予手法治疗,试验组在对照组治疗基础上采取中药封包治疗,两组疗程均为2周。观察比较两组患者临床治疗效果、半月板脱落情况及不良反应发生情况;治疗前后视觉模拟评分(VAS)、Lysholm膝关节功能评分表(LKSS)、Barthel指数(BI)评分、关节活动度(ROM)变化情况。结果102例膝关节的204个内外半月板中,Ⅰ级损伤45个(22.06%),Ⅱ级损伤159个(77.94%);内侧半月板120个(58.82%),外侧半月板84个(41.18%)。治疗后,试验组总有效率94.12%,高于对照组80.39%(P<0.05);两组VAS评分均下降(P<0.05),且试验组低于对照组(P<0.05);两组BI评分均升高(P<0.05),且试验组高于对照组(P<0.05);两组患者LKSS均升高(P<0.05),且试验组高于对照组(P<0.05);两组ROM变化情况和不良反应发生情况比较,差异无统计学意义(P>0.05)。结论中药封包联合手法治疗半月板损伤,可改善患者关节疼痛、肿胀等症状,减轻患者膝关节活动障碍,改善患者日常生活。Objective To investigate the therapeutic effect of Chinese herbal package therapy combined with manual treatment on meniscus injury and its effect on knee function.Methods A total of 102 patients with meniscus injury treated in Sichuan Orthopedic Hospital from January 2019 to December 2020 were enrolled and divided into experimental group(n=51)and control group(n=51)according to random number table method.The control group was given manual treatment,while the experimental group was given traditional Chinese medicine package therapy combined with manual treatment.The treatment in both groups lasted for 2 weeks.Meniscus shedding,clinical efficacy,incidence of adverse reactions,visual analog scale(VAS)score,Lysholm knee score scale(LKSS),Barthel Index(BI),and range of motion(ROM)before and after treatment were observed and compared between the two groups.And adverse reactions were recorded.Results Among the 204 meniscus of 102 knee joints,45(22.06%)were grade I injury and 159(77.94%)were grade II injury,with 120(58.82%)medial meniscus and 84(41.18%)lateral meniscus.After treatment,the total effective rate in the experimental group was 94.12%,which was higher than 80.39%in the control group(P<0.05).VAS score in both groups decreased(P<0.05),and it was lower in the experimental group than in the control group(P<0.05).BI score in both groups increased(P<0.05),and it was higher in the experimental group than in the control group(P<0.05).LKSS increased in both groups(P<0.05),and it was higher in the experimental group than in the control group(P<0.05).There was no significant difference in the changes of ROM and the incidence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of meniscus injury,Chinese herbal package therapy combined with manipulation can improve the symptoms of joint pain and joint swelling,reduce the knee joint movement disorder and improve the daily life of patients.
