摘　　要：目的分析时间-血糖曲线下面积(AUC)联合凝血功能指标对妊娠期糖尿病(GDM)患者围产结局的预测效能。方法回顾性分析2018年3月至2020年3月赣南医学院第三附属医院收治的78例GDM患者的临床资料,根据其围产结局不同将其分为病例组(发生不良围产结局,40例)和对照组(未发生不良围产结局,38例)。比较两组患者的时间-血糖AUC和凝血功能指标(血栓弹力图R值、K值),绘制受试者工作特征(ROC)曲线图,分析时间-血糖AUC与凝血功能相关指标对GDM患者围产结局的预测效能。结果病例组患者的时间-血糖AUC高于对照组,差异有统计学意义(P<0.05);病例组患者的血栓弹力图R值、K值低于对照组,差异有统计学意义(P<0.05)。时间-血糖AUC、血栓弹力图R值、K值AUC均大于参考线(P<0.05),其截断值分别为17.62 mmol·h、6.76 min、1.46 min。结论时间-血糖AUC与凝血功能指标能有效预测GDM患者围产结局,具有较高的临床应用价值。Objective To analyze the predictive efficacy of time-glucose area under the curve(AUC)combined with coagulation function indicators on perinatal outcomes of patients with gestational diabetes mellitus(GDM).Methods The clinical data of 78 patients with GDM admitted to the Third Affiliated Hospital of Gannan Medical College between March 2018 and March 2020 were retrospectively analyzed.According to different perinatal outcomes,they were divided into the case group(with adverse perinatal outcomes,40 cases)and the control group(without adverse perinatal outcomes,38 cases).The AUC of time-glucose and coagulation function indicators(R value and K value of thromboelastic graph)were compared between the two groups,and receiver operating characteristic(ROC)curve was drawn to analyze the predictive efficacy of AUC of time-glucose and coagulation function related indicators on perinatal outcome of GDM patients.Results The AUC of time-glucose of case group was higher than that of the control group,and the difference was statistically significant(P<0.05).The R value and K value of thromboelastogram in the case group were lower than those of the control group,and the differences were statistically significant(P<0.05).The AUC of time-glucose and AUC values of thromboelastogram R value,K value were larger than the reference line(P<0.05),and the cut-off values were 17.62 mmol·h,6.76 min,1.46 min.Conclusion AUC of time-glucose and coagulation function indicators can effectively predict the perinatal outcomes of patients with GDM,and have high clinical application value.
