老年住院患者衰弱与中性粒细胞/淋巴细胞比值、D-二聚体的相关性研究

摘　　要：目的:探讨老年住院患者衰弱与中性粒细胞与淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)、D-二聚体的关系,为老年人衰弱的诊断与干预提供参考。方法:选取≥65岁住院治疗的患者335例进行衰弱评估,测定中性粒细胞计数、淋巴细胞计数、D-二聚体,并收集其临床资料及实验室检查,分析衰弱与NLR、D-二聚体的相关性。结果:根据Fried量表将患者分为正常组63例,衰弱前期组162例,衰弱组110例。三组患者年龄、NLR、凝血酶原活动度、D-二聚体比较,差异有统计学意义(P<0.05),年龄、NLR、凝血酶原活动度、D-二聚体与衰弱的相关系数r分别为0.282、0.215、-0.165、0.267(P<0.05)。Logistic回归分析提示年龄[OR=1.085,95%CI(1.045,1.128)]、NLR[OR=1.348,95%CI(1.080,1.683)]、D-二聚体[OR=1.938,95%CI(1.320,2.845)]是衰弱的影响因素(P<0.05)。结论:衰弱与年龄、NLR、D-二聚体具有相关性,高龄、高NLR、高D-二聚体提示衰弱风险增加。Objective To explore the relationship between frailty and NLR and D-dimer.To Provide reference for the diagnosis and infervention of senile asthenia.Method 335 hospitalized patients of≥65 years of age were selected for frailty assessment,neutrophil count,lymphocyte count,and D-dimer were measured,and their clinical data and laboratory tests were collected.The correlation relationship between decay and NLR and D-dimer was analyzed.Results According to the Fried scale,63 cases were divided into normal group,162 cases in pre-frailty group,and 110 cases in frailty group.Comparison of age,NLR,prothrombin activity,and D-dimer of the three groups of patients was statistically significant(P<0.05).Age,NLR,prothrombin activity,D-dimer and frailty the correlation coefficient r was 0.282,0.215,-0.165,0.267,hespectively(P<0.05).Logistic regression analysis indicated age[OR=1.085,95%CI(1.045,1.128)],NLR[OR=1.348,95%CI(1.080,1.683)],D-dimer[OR=1.938,95%CI(1.320,2.845)]were influencing factors of frailty(P<0.05).Conclusion Frailty is correlated with age,NLR,and D-dimer.Advanced age,high NLR,and high D-dimer indicate an increased risk of frailty.
