比较左束支起搏技术与传统右室间隔起搏技术的辐射剂量

摘　　要：目的比较永久心脏起搏器植入手术中,左束支起搏(LBBP)技术与传统右心室间隔起搏(RVSP)技术的辐射剂量。方法回顾性分析85例永久心脏起搏器植入患者,按照心室起搏方式,分为左束支起搏组(n=50例)和右心室间隔起搏组(n=35例)。比较两组患者辐射相关参数,包括透视时间、空气比释动能(AK)、剂量面积乘积(DAP)、摄影序列数及摄影帧数。同时用Spearman检验分析两组透视时间和空气比释动能AK值、剂量面积乘积DAP值之间的相关性。结果LBBP与RVSP两组,中位透视时间分别为7.81 min(6.29,12.40)及7.18 min(4.5,9.0)(P<0.05),中位DAP值分别为2401.9Gym^(2)(1543.3,4290.2)及1495.6μGym^(2)(818,2086.3)(P<0.05)、中位AK值分别为175.8 mGy(115.9,302.4)及95 mGy(62,127.8)(P<0.05),LBBP组均高于RVSP组,差异有统计学意义。两组的透视时间与DAP值和AK值均为正相关。结论左束支起搏技术的辐射剂量高于传统的右心室间隔起搏技术。Objective To compare the radiation dose of left bundle branch pacing and traditional right ventricular septal pacing during permanent cardiac pacemaker implantation surgery.Methods A total of 85 patients with permanent cardiac pacemaker implantation were retrospectively analyzed.Patients were divided into right ventricular septal pacing RVSP group(n=35 cases)and left bundle branch pacing LBBP group(n=50 cases)according to different ventricular pacing methods.Several radiation-related parameters of the two groups of patients were compared,including fluoroscopy time,air kerma(AK),dose area product(DAP),the number of photographic sequences and photographic frames.The correlation of the radiation dose and fluoroscopy time was analyzed using Spearman correlation statistics.Results The fluoroscopy time of the LBBP group[7.81(6.29,12.40)vs.7.18(4.5,9.0)min,P<0.05],DAP value[2401.9(1543.3,4290.2)vs.1495.6(818,2086.3)μGym^(2),P<0.05],AK value[175.8(115.9,302.4)vs.95(62,127.8)mGy,P<0.05]were all higher than that of the RVSP group,and the differences were statistically significant.The fluoroscopy time of the two groups was positively correlated with DAP and AK.Conclusion The radiation dose of left bundle branch regional pacing is higher than that of traditional right ventricular septal pacing.
