全髋关节置换术治疗老年移位髋臼骨折的临床分析

摘　　要：目的分析应用全髋关节置换术治疗老年移位髋臼骨折的临床疗效。方法将射阳县中医院2017年1月-2020年4月收治的46例老年移位髋臼骨折患者分为对照组与观察组,各23例。对照组给予切开复位内固定治疗,观察组接受全髋关节置换术治疗,评估2组患者的髋关节功能、临床疗效和并发症情况。结果出院时及术后1个月,对照组患者的Postel评分均明显比观察组低,差异具有统计学意义(P<0.05)。观察组患者的治疗优良率为86.96%,显著高于对照组的60.87%,差异具有统计学意义(P<0.05)。对照组患者的并发症发生率为43.48%,明显高于观察组的13.04%,差异具有统计学意义(P<0.05)。结论老年移位髋臼骨折患者采用全髋关节置换术比切开复位内固定治疗能显著提高临床疗效,加速恢复关节功能,提升髋关节屈伸活动度,减少并发症的发生。Objective To analyze the surgical treatment strategy and efficacy of displaced acetabular fractures in the elderly.Methods From January 2017 to April 2020,46 elderly patients with displaced acetabular fractures in our hospital were divided into control group and observation group,with 23 cases in each group.The control group received open reduction and internal fixation,and the observation group received total hip replacement.The hip joint function,clinical efficacy and complications of the two groups of patients were evaluated.Results At discharge and 1 month after surgery,the Postel scores of the control group were significantly lower than those of the observation group,and the data difference between the two groups was statistically significant(P<0.05).The treatment excellent rate of the observation group was as high as 86.96%,which was significantly better than the 60.87%of the control group.The data difference between the two groups was statistically significant(P<0.05).The complication rate of the control group reached 43.48%,which was significantly higher than the observation group's 13.04%.The data difference between the two groups was statistically significant(P<0.05).Conclusion Among the common surgical options for the treatment of elderly patients with displaced acetabular fractures,total hip arthroplasty can significantly improve the clinical efficacy of patients than open reduction and internal fixation,accelerate the recovery of patients'joint function,and improve the patients'hip joints Flexion and extension activities,reduce the occurrence of complications.
