三田汤外洗联合威伐光照射治疗难治性皮肤溃疡临床研究

摘　　要：目的探讨三田汤外洗联合威伐光照射治疗难治性皮肤溃疡的临床疗效及安全性。方法选取广西壮族自治区皮肤病医院收治的难治性皮肤溃疡患者205例,采用随机数字表法分为观察组104例和对照组101例。对照组给予湿润烧伤膏外涂;观察组给予威伐光和自拟三田汤外洗。比较2组患者治疗前后视觉疼痛模拟量表(Visual analogue scale,VAS)评分及皮肤病生活质量指数(Dermatology life quality index,DLQI)评分、创面愈合时间及愈合率、活动自如程度及患者满意度、不良反应发生情况。结果治疗2周后,2组VAS及DLQI评分均较治疗前减少(P <0.05);治疗4周后,观察组VAS评分与对照组比较,差异有统计学意义(P <0.05);治疗4周后,2组DLQI评分与治疗2周后比较,差异有统计学意义(P <0.01);治疗4周后,观察组创面愈合平均时间短于对照组(P <0.05);治疗2、4周后,观察组愈合率高于对照组(P <0.05);治疗4周后,观察组患者活动自如程度好于对照组(P <0.05);治疗4周后,观察组患者满意度96.15%(100/104)高于对照组的85.15%(86/101)(P <0.05);观察组有5例出现轻微红斑,2例轻微灼热感;对照组有3例轻微涂药创口瘙痒。结论三田汤外洗联合威伐光照射治疗难治性皮肤溃疡能缩短创面愈合时间,提高愈合率,同时提高患者活动自如程度及患者满意度,临床安全有效。Objective To investigate the clinical efficacy and safety of Santian decoction combined with WIRA irradiation in the treatment of refractory skin ulcer. Methods a total of 205 patients with refractory skin ulcer were selected from Guangxi Dermatology Hospital and divided into observation group(104 cases) and control group(101 cases) by random number table method. The control group was externally coated with MEBO. The observation group was given WIRA irradiation and self-designed Santian decoction for external washing. Visual analogue Scale(VAS) score and Dermatology life quality Index(DLQI) score, wound healing time and rate, degree of mobility and patient satisfaction, occurrence of adverse reactions were compared between 2 groups before and after treatment. Results After 2 weeks of treatment, VAS and DLQI scores in both groups were decreased compared with those before treatment(P <0.05). After 4 weeks of treatment, VAS score of the observation group was significantly different from that of the control group(P <0.05). After 4 weeks of treatment, DLQI scores of both groups were significantly different from those of 2 weeks(P <0.01). After 4 weeks of treatment, the average time of wound healing in the observation group was shorter than that in the control group(P <0.05). After 2 and 4 weeks of treatment, the healing rate of observation group was higher than that of control group(P <0.05). After 4 weeks of treatment, the activity of observation group was better than control group(P <0.05). After 4 weeks of treatment, the satisfaction of patients in the observation group was 96.15%(100/104) higher than 85.15%(86/101) in the control group(P <0.05). In the observation group, 5 cases had slight erythema and2 cases had slight burning sensation. In the control group, there were 3 cases with mild pruritus. Conclusion Santian decoction external washing combined with WIRA irradiation in the treatment of refractory skin ulcer can shorten the healing time of the wound surface,improve the healing rate, and improve the patient�
