摘 要:目的探究微生态免疫营养剂对脓毒症胃肠功能障碍大鼠肠道菌群及肠道免疫的影响。方法将42只SD雄性大鼠适应性饲养1周后,随机取12只作为正常对照组,其余大鼠采用盲肠结扎法复制脓毒症模型,以存在明显的脓毒症表现,且呈现出高动力循环状态以及持续高代谢反应状态,同时在发病过程中可见血细菌培养及内毒素检测结果阳性为造模成功标准。将模型大鼠随机分为模型对照组和营养组,每组15只。营养组给予微生态免疫营养剂灌胃,标准量为732 kJ/(kg·d),连续7 d;正常对照组大鼠实施假手术处理,大鼠麻醉后,对大鼠行常规开腹手术治疗,找到盲肠后,不做结扎或穿孔处理。模型对照组大鼠未做特殊处理。比较3组大鼠的肠道菌群变化情况、肠道黏膜分泌型IgA(SIgA)水平、血清D-乳酸水平及生存结局。结果模型对照组的拟杆菌纲水平低于营养组及正常对照组,变形菌门水平要高于营养组及正常对照组,营养组的厚壁菌门水平低于模型对照组及正常对照组,差异均有统计学意义(P <0.05);模型对照组的肠道黏膜SIgA水平低于营养组及正常对照组,血清D-乳酸水平高于营养组及正常对照组,差异均有统计学意义(P <0.05);模型对照组的死亡率高于营养组及正常对照组,差异有统计学意义(P <0.05)。结论微生态免疫营养剂可针对脓毒症胃肠功能障碍的肠道菌群及肠道免疫功能等异常情况发挥有利的调节作用。Objective To investigate the effects of microecological immune nutrients on intestinal flora and intestinal immunity in septic rats with gastrointestinal dysfunction. Methods The male SD rats were randomly divided into normal control group, model control group(no special treatment) and nutrition group(treated with microecological immune nutrition) using random number table. Totally30 rats in model control group and nutrition group were performed to induce sepsis, each group 15. Another 12 rats were selected for sham operation as the normal control group. Next, the changes of rat intestinal flora, intestinal mucosa secretion of specific immunoglobulin(SIgA), and serum D-lactic acid level within the three groups were detected. Results The expression level of Bacteroidetes in the model control group was significantly lower than that in nutrition group and normal control group, and the expression level of proteobacteria was significantly higher than that in nutrition group and normal control group. The expression level of Firmicutes in nutrition group was significantly lower than that in model control group and normal control group. The level of SIg A in intestinal mucosa of model control group was significantly lower than that in nutrition group and normal control group, and the level of serum D-lactic acid was significantly higher than that in nutrition group and normal control group(P <0.05). Conclusion Microecological immune nutrients can effectively regulate intestinal flora and intestinal immune dysfunction in sepsis.
