温阳益肾汤治疗老年便秘临床研究

摘　　要：目的观察温阳益肾汤治疗老年便秘的临床疗效。方法将120例老年便秘患者按照随机数字表法分为2组,治疗组60例予温阳益肾汤治疗,对照组60例予乳果糖口服溶液治疗,2组均治疗4周,疗程结束后3个月随访。比较2组疗效;比较2组治疗前后及治疗后3个月便秘症状评分、汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD-17)评分变化。结果治疗组总有效率95.0%(57/60),对照组总有效率81.7%(49/60),治疗组疗效优于对照组(P<0.05)。2组治疗后排便困难、排便时间、排便频率、腹痛坠胀、大便性状评分均较本组治疗前降低(P<0.05);治疗组治疗后3个月排便困难、排便时间、排便频率、腹痛坠胀、大便性状评分均较本组治疗前降低(P<0.05);治疗后、治疗后3个月治疗组排便困难、排便时间、排便频率、腹痛坠胀、大便性状评分均低于对照组同期(P<0.05)。2组治疗后、治疗后3个月HAMA、HAMD-17评分均较本组治疗前降低(P<0.05);治疗组治疗后与治疗后3个月HAMA、HAMD-17评分比较差异均无统计学意义(P>0.05);对照组治疗后3个月HAMA、HAMD-17评分均较本组治疗后升高(P<0.05)。结论温阳益肾汤治疗老年便秘,能改善患者便秘症状,改善焦虑、抑郁状态,临床效果确切,安全可靠。Objective To observe the clinical efficacy of Wenyang Yishen Decoction for senile constipation(SC).Methods Totally 120 SC patients were randomly divided into two groups,60 cases in treatment group were treated with Wenyang Yishen Decoction,and 60 cases in control group were treated with lactulose oral solution.Treated 4 weeks,the follow-up had carried out for 3 months,aiming to compare curative effect,constipation symptoms score,Hamilton Anxiety Scale(HAMA)score,and 17-item Hamilton Depression Rating Scale(HAMD-17)score.Results The total effective rates were better in treatment group than in control group[95.0%(57/60)vs 81.7%(49/60),(P<0.05),respectively].Compared with before treatment,constipation symptoms score(defecation difficulties,defecation time,defecation frequency,abdominal pain and swelling,stool traits)in groups were decreased after treatment(P<0.05),which in treatment group were significantly decreased in the 3th month(P<0.05),and this above scores were decreased in treatment group than in control group after treatment and in the 3th month of treatment(P<0.05).Compared with before treatment,HAMA and HAMD-17 scores in groups were decreased after treatment and in the 3th month of treatment(P<0.05);the difference was no statistically significant in HAMA and HAMD-17 scores in treatment group between after treatment and in the 3th month of treatment(P>0.05);compared with after treatment,HAMA and HAMD-17 scores in control group were increased in the 3th month of treatment(P<0.05).Conclusion For SC patients,Wenyang Yishen Decoction can improve constipation symptoms,anxiety and depression,which has definite clinical effects,with safety and reliable.
