黄鱼清热宣肺方治疗肺大疱切除术后非感染性发热的临床研究

摘　　要：目的观察黄鱼清热宣肺方治疗肺大疱切除术后非感染性发热的临床疗效。方法将50例肺大疱切除术后非感染性发热患者按照随机数字表法分为2组,对照组25例予西医常规治疗,治疗组25例在对照组基础上加用黄鱼清热宣肺方治疗。2组均治疗7 d为1个疗程,治疗2个疗程,比较2组治疗后发热相关指标(包括伴随症状、发热程度、发热持续时间及发热次数)评分、炎症相关因子[包括白细胞介素6(IL-6)、超敏C反应蛋白(hs-CRP)及肿瘤坏死因子α(TNF-α)]水平、肺功能指标[包括用力肺活量(FVC)、FVC占预计值百分比(FVC%)、第1 s用力呼气容积(FEV 1)及FEV 1占预计值百分比(FEV 1%)]及简明健康状况调查表(SF-36)(包括躯体疼痛、生理职能、情感职能、社会功能、精神健康、一般健康、精力及生理机能)评分变化情况。结果治疗组治疗后发热相关指标伴随症状、发热程度、发热持续时间、发热次数评分及总分均低于对照组(P<0.05)。治疗组治疗后炎症相关因子IL-6、hs-CRP及TNF-α水平均低于对照组(P<0.05)。治疗组治疗后肺功能指标FVC、FVC%、FEV 1及FEV 1%水平均高于对照组(P<0.05)。治疗组治疗后SF-36躯体疼痛、生理机能、情感职能、社会功能、精神健康、一般健康、精力及生理职能评分均高于对照组(P<0.05)。结论黄鱼清热宣肺方治疗肺大疱切除术后非感染性发热临床疗效确切,可明显改善患者发热症状,减轻发热程度,减少发热时间,减少发热次数,降低炎症相关因子水平,提高肺功能,改善患者生活质量。Objective To observe the clinical efficacy of HuangyuQingreXuanfei Formula on non-infective fever after bullectomy.Methods Totally 50 patients with non-infective fever afterbullectomy were randomly divided into two groups,25 cases in control group were treated with conventional western medicine,and 25 cases in treatmentgroup additionally with HuangyuQingreXuanfei Formula on the basis of control group.Treated for14 days,the key observation was fever-related indicators(associated symptoms,degree of fever,duration of fever,number of fevers),inflammatory cytokine levels[interleukin-6(IL-6),high-sensitivity c-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)],pulmonary function index[forced vital capacit(FVC),percentage of FVC of predicted value(FVC%),forced expiratory volume in one second(FEV 1),percentage of FEV 1 of predicted value(FEV 1%)],36-item short form health survey(SF-36)(bodily pain,physiological role,emotional role,social function,mental health,general health,vigor and physiological function).Results Compared with control group,treatment group exhibited significantly decreased fever-related indicators(P<0.05),down-regulated inflammatory cytokine levels(P<0.05),increased pulmonary function index(P<0.05),elevated SF-36 score(P<0.05).Conclusion For patients with non-infective fever afterbullectomy,HuangyuQingreXuanfei Formula has definite curative effect,can significantly improve fever symptoms,reduce number of fever and duration of fever,reduce degree of fever and inflammatory cytokine levels,improve lung function and quality-of-life.
