中医外治综合疗法对湿热下注型肛瘘术后止痛促愈的疗效评价

摘　　要：目的观察中医外治综合疗法对湿热下注型肛瘘术后止痛促愈的疗效。方法将68例湿热下注型肛瘘术后患者按照随机数字表法分为2组,治疗组34例予中医外治综合疗法(中药熏洗、化腐清创及耳穴贴压)治疗,对照组34例予西医常规治疗。2组均治疗28 d,比较2组治疗1、4、7、14、28 d疼痛视觉模拟评分(VAS)及疼痛持续时间变化情况,比较2组治疗28 d创面组织愈合情况,比较2组创面完全愈合时间情况。结果治疗组治疗1、4、7、14 d疼痛VAS均明显低于对照组(P<0.05),疼痛持续时间均明显短于对照组(P<0.05),但2组治疗28 d疼痛VAS及疼痛持续时间组间比较差异均无统计学意义(P>0.05)。治疗组治疗28 d创面组织愈合正常率97.06%(33/34),创面完全愈合时间(27.68±3.77)d,对照组分别为73.53%(25/34)、(36.94±4.57)d,治疗组愈合正常率高于对照组(P<0.05),完全愈合所需时间短于对照组(P<0.05)。结论中医外治综合疗法治疗湿热下注型肛瘘术后患者疗效确切,可明显缓解疼痛症状,减少疼痛持续时间,促进创面修复,缩短愈合时间。Objective To evaluate the efficacy Efficacy of comprehensive therapy of traditional Chinese medicine for external use on pain relief and wound healing of postoperative patient with anal fistula(AF)of damp-heat type.Methods A total of 68 postoperative patients with AF of damp-heat type were randomly grouped,34 patients in treatment group were treated with the comprehensive therapy of TCM for external use(herbal fumigation,corruption debridement and auricular point sticking),and 34 patients in control group were treated conventional treatment of western medicine.Treated for 28 days,the key observation was the changes of visual analogue scale(VAS),which were on the 1th,4th,7th,14th,28th day,and pain duration,the superior rate of wound healing on the 28th day,as well as the time to complete wound healing.Results Compared with control group,VAS scores were significantly lower in treatment group on the 1th,4th,7th,14th day(P<0.05);pain duration was significantly shorter on the same period(P<0.05),the difference was no significant in VAS score and pain duration day between groups on the 28th(P>0.05).The superior rate of wound healing was higher in treatment group than in control group on the 28th day[97.06%(33/34)vs 73.53%(25/34),(P<0.05),respectively];the time to complete wound healing was shorter in treatment group than in control group[(27.68±3.77)day vs(36.94±4.57)day,(P<0.05),respectively].Conclusion For postoperative patients with AF of damp-heat type,the comprehensive therapy TCM for external use has a definite effect,which can significantly relieve pain symptoms,reduce pain duration,promote wound repair,and shorten time to wound healing.
