加味补中益气汤治疗肥胖型多囊卵巢综合征不孕的临床研究

摘　　要：目的观察加味补中益气汤治疗肥胖型多囊卵巢综合征(PCOS)不孕的临床疗效。方法将120例肥胖型PCOS不孕患者按照随机数字表法分为2组。对照组60例予西医常规治疗,治疗组60例予加味补中益气汤治疗。2组均以3个月经周期为1个疗程,治疗2个疗程后统计疗效,比较2组治疗后排卵率、妊娠率及排卵日子宫内膜厚度情况,比较2组治疗前后性激素指标[包括睾酮(T)、黄体生成激素(LH)及卵泡刺激素(FSH)]及胰岛素抵抗指标[包括空腹胰岛素(FINS)及空腹血糖(FPG)]水平变化情况,比较2组治疗前后体质量指数(BMI)变化情况。结果治疗组总有效率85.00(51/60),排卵率76.67%(46/60),妊娠率36.55%(22/60),排卵日子宫内膜厚度(10.48±1.87)mm,对照组分别为63.39%(38/60)、73.33%(44/60)、23.33%(14/60)、(7.13±2.22)mm,治疗组总有效率及妊娠率均高于对照组(P<0.05),排卵日子宫内膜厚度大于对照组(P<0.05),但排卵率与对照组相当(P>0.05)。2组治疗后T、LH、FINS及BMI与本组治疗前比较均降低(P<0.05),且治疗组治疗后T、LH、FINS及BMI均低于对照组(P<0.05),2组治疗前后FSH及FPG水平均无明显变化(P>0.05)。结论加味补中益气汤治疗肥胖型PCOS不孕疗效确切,可明显促进患者恢复正常月经周期,改善性激素水平,减轻胰岛素抵抗,降低BMI,提高妊娠成功率。Objective To observe the clinical effect of modified BuzhongYiqi Decoction for infertile patients with obese polycystic ovary syndrome(PCOS).Methods Totally 120 infertile patients with obese PCOS were randomly divided into two group,60 patients in control group were treated with conventional western medicine,and 60 patients in treatment group were treated with modified BuzhongYiqi Decoction.Treated for 6 months,the key observation were ovulation rate,pregnancy rate,endometrial thickness on ovulation day,sex hormone index[testosterone(T),luteinizing hormone(LH),follicle-stimulating hormone(FSH)],insulin resistance index[fasting insulin(FINS),fasting blood glucose(FPG)],as well as body mass index(BMI).Results The total effective rates were better in treatment group than in control group[85.00%(51/60)vs 63.39%(38/60),(P<0.05),respectively];the ovulation rate was 76.67%(46/60)and 73.33%(44/60)in treatment group and control group,respectively,and which in treatment group was equivalent to that in control group(P>0.05);the pregnancy rate in treatment group was higher than that in control group[36.55%(22/60)vs 23.33%(14/60),(P<0.05),respectively];the endometrial thickness on ovulation day was higher in treatment group than in control group[(10.48±1.87)mm vs(7.13±2.22)mm,(P<0.05),respectively].Compared with before treatment,T,LH,FINS and BMI in groups were lower(P<0.05),improvement of which in treatment group were significantly better(P<0.05);there was no significant change in FSH and FPG levels between groups before and after treatment.Conclusion For infertility patients with obese PCOS,modified BuzhongYiqi Decoction has definite curative effect,which can obviously promote patients to return to a normal menstrual cycle,improve sex hormone levels,reduce insulin resistance and BMI level,and improve pregnancy success rate and clinical curative effect.
