达泊西汀联合中等剂量西地那非治疗早泄合并轻度勃起功能障碍的临床效果

摘　　要：目的探讨达泊西汀联合中等剂量西地那非治疗早泄(PE)合并轻度勃起功能障碍(ED)的临床疗效。方法选取2019年4月至2020年9月泉州第一医院泌尿外科男科组诊治的92例PE合并轻度ED患者作为研究对象,遵循随机原则分为观察组(n=47)和对照组(n=45)。观察组同时给予达泊西汀及中等剂量西地那非按需治疗,对照组只予达泊西汀按需治疗,分析两组治疗前后阴道内射精潜伏时间(IELT)、早泄简表(PEP)内4项指标和国际勃起功能指数-5(IIEF-5)评分等。结果在IELT改善方面,用药1个月至3个月,观察组和对照组均较用药前有所改善(P<0.01或P<0.05);观察组用药后1个月的IELT改善较对照组明显(P<0.05);但在用药后2个月及3个月,两组间比较无明显差别(P>0.05)。在PEP内4项指标改善方面,用药后1个月,两组均较用药前显著改善(P<0.01或P<0.05),且观察组射精控制力改善更加明显(P<0.01);而用药后1个月对照组在此方面无进一步改善(P>0.05)。观察组有4例、对照组有2例发生不良反应,均在用药后6~12h逐渐消失。结论达泊西汀联合中等剂量西地那非治疗可早期明显改善PE合并轻度ED患者IELT及射精控制力。Objective To analyze the clinical effect of dapoxetine combined with medium-dose sildenafil in the treatment of premature ejaculation(PE)with mild erectile dysfunction(ED).Methods 92 patients of premature ejaculation with mild erectile dysfunction diagnosed and treated in the outpatient department by the Urology Andrology Group of Quanzhou the First Hospital Affiliated to Fujian Medical University from April 2019 to September 2020 were enrolled.They were randomly assigned to observation group(n=47)and Control group(n=45).The treatment group was treated with dapoxetine and midium-dose sildenafil,while the control group was treated with dapoxetine only.The intravaginal ejaculation latent time(IELT),the four indicators in the premature ejaculation profile(PEP)and the International Index of Erectile Function-5(IIEF-5)scores were analyzed before and after treatment.Results In terms of improvement of IELT,the observation group and the control group were improved after 1 to 3 months medication(P<0.01 or P<0.05).The improvement of IELT in the observation group was more obvious than that in the control group 1 month after medication(P<0.05).However,there was no significant difference between the two groups at 2 and 3 months after medication(P>0.05).In terms of the improvement of 4 indicators in PEP,1 month after medication,both groups were significantly improved compared with before medication(P<0.01 or P<0.05),and the improvement of ejaculation control force in observation group was more obvious(P<0.01).However,there was no further improvement in the control group one month after medication(P>0.05).Adverse reactions occurred in 4 cases in the observation group and 2 cases in the control group,all of which disappeared gradually 6-12 h after administration.Conclusions Dapoxetine combined with medium sildenafil significantly improved IELT and ejaculation control in PE patients with mild ED at early stage.
