摘 要:目的探讨男性特发性少精症、弱精症患者精液外泌体miR-34c表达水平及其临床意义。方法选取2019年6月至12月武汉仁爱医院生殖中心诊治的58例特发性弱精症(弱精组)和44例特发性少精症(少精组)男性不育症患者作为研究对象。另选取同期武汉仁爱医院收取的50例进行精液检查的健康男性作为对照组。比较三组精液外泌体miR-34c、精浆丙二醛(MDA)、一氧化氮(NO)水平、总抗氧化能力和常规精液参数[pH、精子浓度、精子活力PR级(A+B)、精子存活率]。结果与对照组比较,少精组、弱精组精液外泌体miR-34c与T-AOC水平均显著降低,差异具有统计学意义(F=53.140、24.090,P<0.001);少精组、弱精组MDA、NO水平均显著高于对照组,差异具有统计学意义(F=69.060、11.020,P<0.001)。少精组、弱精组精液外泌体miR-34c与MDA、NO水平呈现负相关(r=-0.435、-0.411,P<0.05),与T-AOC、精子浓度、精子活力PR级(A+B)、精子存活率呈现正相关(r=0.526、0.629、0.387、0.462,P<0.05)。在特发性少精症、弱精症患者中,MDA水平与NO水平呈正相关(r=0.781,P<0.05),同时MDA、NO与T-AOC、精子浓度、精子活力PR级(A+B)、精子存活率均呈现负相关(r=-0.650、-0.347、-0.424、-0.615,-0.488、-0.289、-0.430、-0.472,P<0.05);T-AOC与精子浓度、精子活力PR级(A+B)、精子存活率呈现正相关(r=0.406、0.458、0.743,P<0.05)。结论特发性少精症、弱精症患者精液外泌体miR-34c表达低于正常水平,而且与精液氧化应激状态有关。Objective To investigate the expression level and clinical significance of miR-34 c in semen exosomes of patients with male idiopathic oligospermia and asthenospermia.Methods 102 patients with idiopathic male infertility treated in Reproductive Center of Wuhan Ren′ai Hospital from June to December 2019 were recruited as research subjects,including 58 cases of asthenospermia and 44 cases of oligospermia.In addition,50 healthy men who underwent semen examination in Wuhan Ren′ai Hospital during the same period were selected as the control group.Compare the semen exosomes miR-34 c,seminal plasma malondialdehyde(MDA),nitric oxide(NO),total antioxidant capacity and conventional semen parameters[pH,sperm concentration,sperm motility PR grade(A+B),sperm survival rate].Results Compared with the control group,the levels of miR-34 c and T-AOC in semen exosome of oligospermia group and asthenospermia group were significantly decreased(F=53.140,24.090,P<0.001).MDA and NO levels in oligospermia group and asthenospermia group were significantly higher than those in control group(F=69.060,11.020,P<0.001).There was negative correlation between miR-34 c and MDA and NO levels(r=-0.435,-0.411,P<0.05),and positive correlation between miR-34 c and T-AOC,sperm concentration,sperm motility PR grade(A+B),and sperm survival rate(r=0.526,0.629,0.387,0.462,respectively)(P<0.05).In idiopathic oligospermia and asthenospermia,MDA level was positively correlated with NO level(r=0.781,P<0.05).Meanwhile,MDA and NO were negatively correlated with T-AOC,sperm concentration,sperm motility PR grade(A+B)and sperm survival rate(r=-0.650,-0.347,-0.424,-0.615,-0.488,-0.289,-0.430,-0.472,P<0.05).T-AOC was positively correlated with sperm concentration,sperm motility PR grade(A+B)and sperm survival rate(r=0.406,0.458,0.743,P<0.05).Conclusions The expression of miR-34 c in semen exosomes of idiopathic oligospermia and asthenospermia is lower than normal,and is related to the status of semen oxidative stress.
