激素替代治疗对更年期综合征患者的临床效果及对治疗依从性的影响因素分析

摘　　要：目的探讨激素替代治疗女性更年期综合征的疗效及对患者治疗依从性的影响因素。方法选取2018年8月至2019年8月唐山市人民医院诊治的90例更年期综合征患者作为研究对象。所有患者均给予激素替代治疗,治疗后评估临床疗效,分析治疗依从性的影响因素。结果依从性好组治疗总有效率显著高于依从性差组,差异具有统计学意义(P<0.05)。治疗后,患者雌二醇(E_(2))水平较治疗前升高,卵泡刺激素(FSH)、黄体生成素(LH)水平较治疗前降低,且依从性好组E_(2)水平高于依从性差组,FSH、LH水平低于依从性差组,差异具有统计学意义(P<0.05)。经Logistic回归分析显示,文化程度、经济收入、婚姻状况、疾病认知程度、家庭支持、治疗费用、不良反应、疾病程度以及医患关系等是影响更年期综合征患者治疗依从性的危险因素(OR>1,P<0.05)。结论激素替代治疗是更年期综合征的有效治疗方案,文化程度、收入、婚姻状况、疾病认知程度、家庭支持、医疗费用、不良反应、疾病程度以及医患关系等是影响患者治疗依从性的危险因素。Objective To explore the effect of hormone replacement therapy on female menopausal syndrome and the influencing factors of patient compliance.Methods The 90 patients with menopausal syndrome admitted to Tangshan People's Hospital from August 2018 to August 2019 were selected as the research objects.All patients were given hormone replacement therapy.After the treatment,the clinical efficacy was evaluated and the factors affecting treatment compliance were analyzed.Results The total effective rate of treatment in the good compliance group was higher than the poor compliance group,with statistical significance(P<0.05).After treatment,E_(2) level was higher than before treatment,FSH and LH levels were lower than before treatment,and E_(2) level in the good compliance group was higher than that in the poor compliance group,while FSH and LH levels were lower than that in the poor compliance group,with statistical significance(P<0.05).Logistic regression analysis showed that education level,economic income,marital status,disease awareness,family support,treatment costs,adverse reactions,disease degree and doctor-patient relationship were the risk factors affecting the treatment compliance of patients with menopause syndrome(OR>1,P<0.05).Conclusions Hormone replacement therapy is an effective treatment for menopausal syndrome.Education level,income,marital status,cognition,family support,medical expenses,adverse reactions,disease degree and doctor-patient relationship are risk factors affecting patients′treatment compliance.
