摘　　要：目的探讨孕妇免疫球蛋白G(IgG)效价与ABO血型不合致新生儿溶血病(HDN)的相关性。方法选取2018年1月至2020年1月南京医科大学附属妇产医院收治的952例进行产检并分娩的孕妇作为研究对象。孕妇从16周进行血清IgG型抗A(B)抗体效价监测,分娩后检测新生儿血型,对A、B型血新生儿进行游离IgG抗体试验、直接抗人球蛋白试验、抗体释放试验。分析孕妇血清IgG型抗A(B)抗体效价与ABO血型不合致HDN发生及HDN严重程度的相关性。结果1次妊娠孕妇IgG抗体阳性率显著低于≥2次妊娠孕妇,HDN组孕妇的血清IgG型抗A(B)抗体效价高于非HDN组孕妇,且HDN发生率随着IgG抗体效价的增加而呈增加趋势。结论孕妇血清IgG型抗A(B)抗体效价能在一定程度上预测新生儿HDN的发生。Objective To investigate the correlation between immunoglobulin G(IgG)titer in pregnant women and hemolytic disease of newborn(HDN)caused by ABO blood group incompatibility.Methods 952 pregnant women who underwent labor check-up and delivered in the Obstetrics and Gynecology Hospital Affiliated to Nanjing Medical University from January 2018 to January 2020 were selected as the research subjects.The titer of serum IgG type anti-A(B)antibody was monitored from 16 weeks of pregnant women.Blood type of newborns was detected after delivery.Free IgG antibody test,direct anti-human globulin test and antibody release test were performed on newborns with blood type A and B.To analyze the correlation between the titer of IgG type anti-A(B)antibody and the occurrence and severity of HDN caused by ABO blood group incompatibility in pregnant women.Results The positive rate of IgG antibody in pregnant women with one pregnancy was significantly lower than that in pregnant women with more than two pregnancies.The serum IgG type anti-A(B)antibody titer in pregnant women of HDN group was higher than that in non-HDN group,and the incidence of HDN increased with the increase of IgG antibody titer.Conclusions The titer of IgG type anti-A(B)antibody in pregnant women can predict the occurrence of HDN to a certain extent.
