摘　　要：目的评价同种异体输血相关医院感染的危险因素,为有针对性地进行医院感染控制提供依据。方法对2019年1月-2020年12月简阳市人民医院住院患者进行回顾性队列研究,使用COX比例风险模型和Kaplan-Meier方法对同种异体输血和医院感染的关联情况进行分析。结果输血患者医院感染率6.92%,高于未输血患者的1.20%,差异有统计学意义(χ^(2)=1808.887,P<0.001);与输注红细胞比较,冷沉淀因子、血浆的感染风险差异无统计学意义,血小板的感染风险较高HR=2.391(95%CI:2.232~4.852)。结论同种异体输血是医院感染的独立危险因素,其中血小板输注的医院感染风险较高,医院感染防控领域应对此高度重视,弥补可能存在的缺失和疏漏。OBJECTIVE To evaluate the risk factors for allogeneic blood transfusion so as to take targeted measures for control of nosocomial infection.METHODS A retrospective cohort study was conducted for the patients who were hospitalized in Jianyang People′s Hospital from Jan 2019to Dec 2020.The association between the allogeneic blood transfusion and the nosocomial infection was analyzed by using COX proportional hazard model and Kaplan-Meier method.RESULTS The incidence of nosocomial infection of the patients with blood transfusion was 6.92%,significatly higher than 1.20%of the patients without blood transfusion(χ^(2)=1808.887,P<0.001).As compared with transfusion of red blood cells,there was no significant difference in the risk for infection between cryoprecipitate factor and plasma,but the risk for infection of platelets was increased HR=2.391(95%CI:2.232~4.852).CONCLUSION Allogeneic blood transfusion is an independent risk factor for nosocomial infection,the patients with transfusion of platelets are at higher risk of nosocomial infection.It is necessary to attach great importance to the prevention and control of nosocomial infection and make up for the possible deficiencies and omissions.
