摘  要:目的分析慢性阻塞性肺疾病(COPD)合并肺部感染的危险因素及患者血浆长链非编码RNA(LncRNA)核丰富转录本1(NEAT1)表达水平变化。方法选择2018年1月-2020年1月江南大学附属医院收治的COPD患者300例,分析患者合并肺部感染情况及血浆LncRNA NEAT1表达水平变化,采用多因素Logistic回归分析COPD合并肺部感染的危险因素,采用受试者工作特征曲线(ROC)评价血浆LncRNA NEAT1对COPD合并肺部感染的诊断价值。结果300例COPD患者共122例出现肺部感染,发生率为40.67%;感染患者痰液培养共检出病原菌97株,其中革兰阳性菌占36.08%,革兰阴性菌占61.86%,真菌占2.06%;COPD合并肺部感染组患者血浆LncRNA NEAT1相对表达水平高于未合并肺部感染组;多因素Logistic回归分析结果,合并糖尿病、长期使用抗菌药物、血浆LncRNA NEAT1是COPD合并肺部感染的危险因素;ROC曲线分析结果,血浆LncRNA NEAT1诊断COPD合并肺部感染曲线下面积为0.887,敏感度为84.40%,特异度为91.60%。结论COPD合并肺部感染患者血浆LncRNA NEAT1表达水平升高,监测血浆LncRNA NEAT1表达水平有助于提高对COPD合并肺部感染的诊断。OBJECTIVE To explore the risk factors for pulmonary infection in chronic obstructive pulmonary disease(COPD)patients and observe the change of expression level of plasma long non-coding RNA(LncRNA)nuclear enriched abundant transcript 1(NEAT1).METHODS A total of 300patients with COPD who were treated in the Affiliated Hospital of Jiangnan University from Jan 2018to Jan 2020were enrolled in the study,the incidence of pulmonary infection and change of plasma LncRNA NEAT1level were observed,multivariate logistic regression analysis was performed for risk factors for the pulmonary infection in the COPD patients,and the value of plasma LncRNA NEAT1in diagnosis of pulmonary infection was evaluated by means of receiver operating characteristic(ROC)curve.RESULTS Of the 300COPD patients,122had pulmonary infection,with the incidence rate 40.67%.Totally 97strains of pathogens were isolated from sputum culture of the patients with infection,36.08%of which were gram-positive bacteria,61.86%were gram-negative bacteria,and 2.06%were fungi.The relative expression level of plasma LncRNA NEAT1of the COPD patients complicated with pulmonary infection was significantly higher than that of the COPD patients without pulmonary infection.Multivariate logistic regression analysis showed that complication with diabetes mellitus,long-term use of antibiotics and plasma LncRNA NEAT1 were the risk factors for pulmonary infection in the COPD patients.ROC curve analysis indicated that the area under curve of the plasma LncRNA NEAT1was 0.887in diagnosis of pulmonary infection,with the sensitivity 84.40%,the specificity 91.60%.CONCLUSIONThe expression level of plasma LncRNA NEAT1of the COPD patients complicated with pulmonary infection is elevated,and the monitoring of plasma LncRNA NEAT1may facilitate the diagnosis of pulmonary infection in the COPD patients.
