李杲对脏气法时升降浮沉补泻理论的建构

摘　　要：李杲汲取《黄帝内经》《难经》、张元素等诸家脏腑用药补泻理论,吸收同时代思想文化,在对医学理论的体悟和临床实践的总结观察之上,有所创新和发挥。认为脾胃为气血阴阳生化之根蒂,是其它四脏气机升降浮沉的基础。值得注意的是,五脏气机的运行变化从《黄帝内经》时期“生长收藏”悄然转变为“升降浮沉”,从而建构出新的脏腑补泻法式,便是脏气法时升降浮沉补泻理论。其在遣方用药中,从脾胃立论,创建补中益气汤、升阳益胃汤等千古名方。此理论的形成,使脏腑补泻理论为之一变,为金元以后的医家遣方用药发展奠定了基础,对中医理论的发展做出重要贡献。LI Gao drew from the Huangdi Neijing,Nanjing,ZHANG Yuan-su and other Zang-Fu reinforcement-reduction medication theory,absorbed the contemporary ideology and culture,innovated and played on the basis of understanding of medical theory and summary observation of clinical practice.It was believed that the spleen and stomach are the root of the biochemistry of qi-blood-yin-yang,and the basis of the rise and fall of the other four internal qi function.It was important to note that the operation of the five Zang-organs qi activity changes from Huangdi Neijing period‘generation-growth-gathering-storage’quietly into‘ascendingdescending-floating-sinking’,thus constructing a new Zang-fu reinforcement-reduction method,Zangqi-Fashi,ascending-descendingfloating-sinking,and reinforcement-reduction.From the spleen and stomach theory in medication,famous formula through the ages were created,such as Buzhong Yiqi Decoction,Shengyang Yiwei Decoction.The formation of the theory made a new look on thoughts of Zang-fu reinforcement and reduction to look brand-new,laid the groundwork for Jinyuan medication theory development,to make important contributions to the development of traditional Chinese medicine theory.
