温胆汤合金匮肾气丸加减治疗老年高血压病合并抑郁的临床疗效观察

摘　　要：目的:探讨温胆汤合金匮肾气丸加减治疗老年高血压病合并抑郁的临床疗效。方法:将90例老年高血压病合并抑郁患者随机分为观察组(46例)和对照组(44例),两组均采用高血压病内科常规治疗,在此基础上对照组服用盐酸帕罗西汀片,观察组服用温胆汤合金匮肾气丸加减治疗,疗程均为12周,治疗前后记录各观察指标。结果:治疗后,观察组综合疗效总有效率显著高于对照组(P<0.01)。两组脉压差都有不同程度缩小,观察组收缩压显著下降(P<0.05),舒张压显著上升(P<0.05),两组比较,差异有统计学意义(P<0.05)。两组治疗4周及结束时HAMD评分较治疗前均降低(P<0.01),观察组显著优于对照组(P<0.01)。两组患者SF-36积分不同程度提高,观察组GH、RE、MH优于对照组(P<0.05)。治疗后两组血脂脂标均有改善,其中,治疗组Lp(a)、HDL-C改善优于对照组(P<0.05)。结论:西医综合治疗联合温胆汤合金匮肾气丸加减治疗老年高血压病合并抑郁,能降低患者血压,改善抑郁症状,提高患者生活质量。Objective:To investigate the clinical effect of Wendan Decoction combined with Jinkui Shenqi Pills in the treatment of senile hypertension with depression.Methods:A total of 90 patients with senile hypertension and depression were randomly divided into observation group(46 cases)and control group(44 cases).Both groups received routine medical treatment for hypertension.On this basis,the control group received Paroxetine Hydrochloride Tablets,and the observation group took Wendan Decoction and Jinkui Shenqi Pills.The course of treatment was 12 weeks,and all the observation indexes were recorded before and after treatment.Results:After treatment,the combined effective rate of the observation group was significantly higher than that of the control group(P<0.01).The difference in pulse pressure between the two groups decreased to varying degrees.The systolic pressure in the observation group significantly decreased(P<0.05),while the diastolic pressure significantly increased(P<0.05),and there was a statistical difference between the two groups(P<0.05).HAMD scores in both groups were decreased,and the observation group was significantly better than the control group(P<0.01).The reduction of HAMD score in the observation group at 4 weeks of treatment was better than that in the control group at the same period(P<0.01),indicating that the effect was faster in the observation group.The score of SF-36 of the health measurement scale of patients in the two groups was improved in different degrees.The observation group was significantly better than the control group(P<0.01).The quality of life after treatment was improved better in the control group.The lipid mass spectrometry of atherosclerosis was improved in both groups after treatment.Compared with the control group,Lp(a)and HDL-C treatment group was superior to the control group(P<0.05).Conclusion:The comprehensive treatment of western medicine combined with the Wendan Decoction and Jinkui Shenqi Pills in the treatment of senile hypertension with depression is effectiv
