外侧脚深面交叉支撑移植物矫治鼻翼退缩的临床疗效

摘　　要：目的探讨利用外侧脚深面交叉支撑移植物改善鼻翼退缩的临床疗效。方法自2015年1月至2019年12月,成都高新心悦容医疗美容门诊部美容外科对16例靠近鼻翼中外侧的重度鼻翼退缩患者,采用自体肋软骨制作鼻翼软骨外侧脚深面交叉支撑移植物进行矫正。将鼻翼软骨穹窿缝合固定于鼻尖的支撑结构,松解部分鼻翼软骨外侧脚与前庭黏膜形成囊袋,将雕刻好的肋软骨移植物插入,下推外侧脚,将头端内侧缝合固定于鼻中隔前角背侧,尾端可适当超过外侧脚尾侧,移植物中部与外侧脚褥式缝合固定。结果16例患者获随访12~36个月;切口均愈合良好,双侧鼻翼对称、形态自然、瘢痕不明显。除2例患者因矫正不足经修复后满意外,余者均对效果满意,满意率为87.5%,无一例感染、鼻翼缘塌陷、夹捏畸形、通气功能障碍等并发症发生。结论采用自体肋软骨行外侧脚深面交叉支撑移植的方法矫正重度鼻翼退缩可获得良好的疗效。Objective To explore the clinical efficacy of lateral crus deep cross support grafts in the correction of alar retraction.Methods From January 2015 to December 2019,16 patients with severe alar retraction close to the middle and lateral sides of the alar rim were collected.The autologous costal cartilage was used to make lateral crus deep cross support grafts for correction.The alar cartilage dome was sutured and fixed to the nasal tip support structure,the lateral crus of alar cartilage was partially loosened to form a sac with the vestibular mucosa.The carved costal cartilage was inserted and the lateral crus was pushed down.The tip position of the graft was sutured and fixed on the dorsal anterior septal angle,the end was appropriately sutured over the tail of the lateral crus and the middle part of the graft was sutured and fixed on the lateral crus through mattress suture.Results Sixteen patients were followed up for 12 to 36 months.All incisions healed well.The bilateral alar was symmetrical,the morphology was natural and the scar was not obvious.Fourteen patients achieved satisfactory results,and 2 patients were satisfied with the results after secondary correction.The satisfaction rate was 87.5%.No infection,alar rim collapse,pinch deformity,ventilatory dysfunction and other complications occurred.Conclusion Good therapeutic effect can be achieved by using the lateral crus deep cross support grafts in the correction of severe alar retraction.
