闭合式入路自体肋软骨颗粒移植隆鼻术的临床效果观察

摘　　要：目的探讨采用闭合式入路行自体肋软骨颗粒移植隆鼻手术的美学效果的优势。方法回顾自2019年6月至2020年9月,钦州市第一人民医院烧伤整形外科收治的86例要求行鼻整形手术患者的临床资料,将接受闭合式入路自体肋软骨颗粒移植隆鼻术的44例患者设为观察组;接受膨体聚四氟乙烯假体置入隆鼻术的42例患者设为对照组。比较两组手术前后鼻面角、鼻额角、鼻尖凸度、鼻唇角的变化和统计学意义,以及两组术后并发症的发生率和患者的满意度。结果术后6个月随访时,两组的鼻唇角、鼻面角比较其差异无统计学意义(P>0.05);而鼻额角和鼻尖凸度观察组较对照组改善明显,其差异具有统计学意义(P<0.05);并发症的发生率两组的差异无统计学意义(P>0.05)。术后3个月和6个月随访时,观察组患者的满意度明显高于对照组,其差异具有统计学意义(P<0.05)。结论采用闭合式入路行自体肋软骨颗粒移植隆鼻术具有塑造鼻背、鼻尖形态,可达到与e-PTFE假体同样支撑力的优势,且患者满意度较高,值得临床推广应用。Objective To explore the advantages of aesthetic effects of autologous diced costal cartilage for rhinoplasty through closed approach.Methods The clinical data of 86 patients underwent rhinoplasty between June 2019 and September 2020 were retrospectively analyzed.In the observation group,44 patients underwent autologous diced costal cartilage for rhinoplasty through closed approach.In the control group,42 patients underwent e-PTFE prosthesis and auricular cartilage for rhinoplasty.The changes of naso-frontal angle,nasolabial angle,nasal surface angle and nasal tip projection before and after operation between 2 groups were compared.The incidence rate of postoperative complications and patients’satisfaction between 2 groups were compared.Results There were no significant differences in nasolabial angle and nasal surface angle between 2 groups at 6 months of follow-up(P>0.05).The improvement of naso-frontal angle and nasal tip projection of observation group was better than that of control group(P<0.05).There was no significant difference in the incidence rate of postoperative complications between 2 groups(P>0.05).At 3 months and 6 months of follow-up,the satisfaction rate of observation group was significantly higher than that of control group(P<0.05).Conclusion The application of autologous diced costal cartilage for rhinoplasty through closed approach has the advantages of small incision,low incidence rate of complications and concealed scar.The patients’satisfaction is high.It should be considerable for clinical promotion and application.
