近腋前后线微创小切口搔刮腋臭根治术的临床应用

摘　　要：目的观察近腋前线、腋后线两种微创小切口入路搔刮根治腋臭的临床效果。方法自2018年4月至2020年5月,衡水市人民医院烧伤整形外科收治148例(296侧)中、重度腋臭患者,采用随机数字表法分为A、B两组,每组74例(148侧)。A组采用近腋后线、B组采用近腋前线切口入路搔刮治疗。记录每例患者手术完成时间、治疗效果,以及术后并发症情况,包括局部血肿、切口黑紫或坏死、皮片坏死等。结果 148例患者术后获随访≥5个月,效果均较好。总手术时长:A、B组分别为(58.0±2.0)min、(58.2±2.0)min;并发症情况:A组皮片坏死1例(双侧),B组切口边缘坏死和发生血肿各1例(单侧);治愈率:A组为97.29%,B组为96.62%。两组比较其差异均无统计学意义(P>0.05)。结论采用近腋前线、腋后线微创小切口入路搔刮根治腋臭,具有操作简单便捷、创伤较小、并发症较少、无明显瘢痕等优点,术后效果良好。Objective To observe the curative effect ofcurettage through two kinds ofmini incision approaches to anterior or posterior line for axillary osmidrosis. Methods From April 2018 to May 2020, 148 patients(296 sides) with moderate to severe axillary osmidrosis were selected. All patients were divided into 2 groups according to random numbers table, 74 patients(148 sides) in each group.The curettage through mini incision approach to near the posterior axillary line was performed in group A and near the anterior axillary line was performed in group B. The completion time of operation, curative effect and postoperative complications such as local hematoma, incision necrosis and skin necrosis were observed in each patient. Results All patients were followed up for more than 5 months after operation. The curative effect was good. Total operation duration was(58.0±2.0) min in group A and(58.2±2.0) min in group B. There was 1 case of skin necrosis(bilateral sides) in group A, 1 case of incision margin necrosis and 1 case of hematoma(unilateral side) in group B. The cure rate was 97.29% in group A and 96.62% in group B. There was no statistical difference between 2 groups(P>0.05). Conclusion The curettage through mini incision approach to anterior or posterior axillary line for axillary osmidrosis has the advantages of simple and rapid operation, less trauma and complications. The scar is not obvious and the postoperative curative effect is good.
