595nm可调脉宽脉冲染料激光联合超声波导入喜辽妥治疗烧伤后增生性瘢痕疗效分析

摘　　要：目的:研究595nm可调脉宽脉冲染料激光联合超声波导入喜辽妥治疗烧伤后增生性瘢痕的疗效及对VAS评分和DLQI评分的影响。方法:本研究采取前瞻性研究方法,以笔者医院2017年1月-2020年1月诊治的120例烧伤后增生性瘢痕患者为研究对象,按照随机分组原则,分为观察组和对照组,每组60例。两组患者均采取超声波导入喜辽妥治疗,观察组采取595nm可调脉宽脉冲染料激光联合治疗,两组患者均治疗2周,比较两组的治疗效果、VAS评分、VSS评分、DLQI评分以及不良反应之间的差异。结果:观察组治疗总有效率显著高于对照组,具有统计学意义(P<0.05);治疗后,观察组2周、1年的VAS评分、瘢痕柔软度、色泽、厚度、血管分布、总分、自我感知、个人社交、家庭生活以及总分显著低于对照组;两组患者发生皮肤灼痛、轻度瘀斑的差异比较,无统计学意义(P>0.05)。结论:595nm可调脉宽脉冲染料激光联合超声波导入喜辽妥治疗烧伤后增生性瘢痕患者,VAS评分及DLQI评分显著下降,治疗效果较好,建议临床推广。Objective To study the effect of 595nm pulsed dye laser with adjustable pulse width combined with ultrasound on VAS score and DLQI score of hypertrophic scar after burn.Methods In this study,120 patients with hypertrophic scar after burn diagnosed and treated in our hospital from January 2017 to January 2020 were selected as the research objects.According to the principle of random grouping,the above patients were randomly divided into the observation group and the control group,with 60 cases in each group.All patients were treated with ultrasound-guided Xiliaotuo,and the observation group was treated with 595nm adjustable pulse width The two groups were treated for 2 weeks.The therapeutic effect,VAS score,VSS score,DLQI score and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was significantly higher than that of the control group.After treatment,the VAS score,scar softness,color,thickness,vascular distribution,total score,self perception,personal social,family life and total score of the observation group were significantly lower than those of the control group at two weeks and one year after treatment;There was no significant difference in skin burning pain and mild ecchymosis between the two groups(P>0.05).Conclusion The VAS score and DLQI score of 595nm adjustable pulse width dye laser combined with ultrasound in the treatment of hypertrophic scar after burn were significantly decreased,and the treatment effect was good,so it is recommended to be popularized in clinic.
