摘 要:目的 探讨血清D-二聚体(D-D)、微小核糖核酸(mi R)-92a表达水平与老年糖尿病患者肾功能损伤的相关性。方法 选取2020年1月-4月江西省兴国县人民医院115例老年糖尿病患者作为研究对象,统计患者肾功能损伤发生情况,根据是否存在肾功能损伤分为糖尿病肾病(DN)组(存在肾功能损伤)和对照组(不存在肾功能损伤),检测2组血清D-D、mi R-92a表达水平对老年糖尿病患者肾功能损伤的影响与预测价值。结果 115例老年糖尿病患者中,21例发生肾功能损伤,发生率为18. 26%(21/115);DN组血清空腹血糖(FPG)、餐后2小时血糖(2 h PG)、D-D、mi R-92a均高于对照组,差异有统计学意义(P <0. 05);经二元Logistic回归分析后建立多元回归模型行多因素分析,结果显示,血清D-D、mi R-92a过表达是老年糖尿病患者肾功能损伤的影响因素(OR> 1,P <0. 05);绘制ROC曲线发现,血清D-D、mi R-92a预测老年糖尿病患者肾功能损伤风险的AUC均> 0. 80。结论 老年糖尿病患者有较高的肾功能损伤风险,可能与血清D-D、mi R-92a过表达有关,早期检测患者血清D-D、mi R-92a水平可预测肾功能损伤风险,可能对指导早期干预、改善患者预后有积极意义。Objective To investigate the correlation between serum D-dimer(D-D)and microribonucleic acid(miR)-92a levels and renal function damage in elderly patients with diabetes mellitus.Methods A total of 115 elderly patients with diabetes mellitus from Xingguo County People's Hospital during January-April 2020 were selected as the research objects.The occurrence of renal impairment in patients was counted,and all the cases were divided into a diabetic nephropathy(DN)group(with kidney damage)and a control group(without kidney damage)according to whether kidney damage occurred.The effect and predictive value of serum D-D and miR-92a levels on renal function injury in elderly diabetic patients were detected in the two groups.Results Among 115 elderly patients with diabetes mellitus,21 had renal function impairment,the incidence rate was 18.26%(21/115);the serum fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2hPG),D-D and miR-92a of the DN gioup were higher than those of the control group(P<0.05).after binary Logistic regression analysis,a multiple regression model was established and multi-factor analysis was performed.The results showed that overexpression of serum D-D and miR-92a was an influencing factor of renal function damage in elderly patients with diabetes mellitus,with the differences statistically significant(OR>1,P<0.05);the ROC curve was drawn and found that serum D-D and miR-92a predicted the AUC of the risk of renal damage in elderly patients with diabetes mellitus were both greater than 0.80.Conclusion Elderly patients with diabetes mellitus have higher renal function impairment,which may be related to the overexpression of serum D-D and miR-92a.Early detection of serum D-D and miR-92a levels in patients can predict the risk of renal damage,and may have positive significance for guiding early intervention and improving the prognosis of patients.
