摘 要:目的比较上腹部双能虚拟平扫(Virtual Non-contrast-Enhanced,VNC)和传统真实CT平扫(True Non-contrastEnhanced,TNC)的图像质量及辐射剂量,探讨虚拟平扫代替传统平扫的可行性。方法应用双源CT对37例患者行上腹部增强扫描,其中平扫采用单源模式,动脉期和静脉期采用双能量模式,通过后处理软件Liver-VNC处理得到动脉期和门静脉期虚拟平扫图像(VNC_(A)、VNC_(V)),比较TNC、VNC_(A)、VNC_(V)三组图像中肝脏、脾脏、胰腺、竖脊肌的CT值、信噪比(Signal to Noise Ratio,SNR)、图像评分、病变检出率以及辐射剂量。结果肝、脾、胰腺、竖脊肌在TNC、VNC_(A)、VNC_(V)三期图像的CT值、SNR、噪声比较,差异有统计学意义(P<0.05);而VNC_(A)、VNC_(V)之间CT值(肝P=0.151;脾P=0.644;胰腺P=0.399;竖脊肌P=0.558)、SNR(肝P=0.568;脾P=0.922;胰腺P=0.628;竖脊肌P=0.745)、噪声(肝P=0.856;脾P=0.983;胰腺P=0.515;竖脊肌P=0.745),差异无统计学意义;TNC、VNC_(A)、VNC_(V)三期图像辐射剂量,差异无统计学意义(P=0.286),而传统增强扫描模式与双能扫描模式总的辐射剂量比较,差异有统计学意义(P<0.001);TNC、VNC_(A)、VNC_(V)三期图像的图像评分,差异无统计学意义(P=0.701);TNC、VNC_(A)、VNC_(V)三期图像的病变检出率分别为82.2%、81.5%、80.8%,差异无统计学意义(χ^(2)=0.091,P=0.956)。结论上腹部虚拟平扫基本上可以代替常规平扫,还能减少一期扫描剂量,在回访病人中能发挥很大优势。Objective To compare the image quality of dual-energy virtual non-contrast-enhanced(VNC)and true noncontrastenhanced CT image(TNC)of the upper abdomen,and explore the feasibility of VNC instead of TNC.Methods Dual-energy CT was used to perform contrast-enhanced scans of upper abdomen in 37 patients,in which the plain scan was in single-energy mode,and dual-energy mode was used for arterial and venous phase scanning.The post-processing software Liver-VNC was used to obtain VNC of arterial and portal venous scan images(VNC_(A),VNC_(V)),compared with CT value,signal to noise ratio(SNR),image score,lesion detection rate and radiation dose of liver,spleen,pancreas and erector spinae muscle in the three groups of TNC,VNC_(A),and VNC_(V)images.Results There were significant difference between the CT values,SNR and noise of liver,spleen,pancreas and erector spinae between TNC and VNC_(A)and VNC_(V)(P<0.05).And there was no significant difference between VNC_(A)and VNC_(V),the CT values between VNC_(A)and VNC_(V)(liver P=0.151;spleen P=0.644;pancreas P=0.399;erector spinae P=0.558),SNR(liver P=0.568;spleen P=0.922;pancreas P=0.628;verticalSpinal muscle P=0.745),noise(liver P=0.856;spleen P=0.983;pancreas P=0.515;erector spinae P=0.745);there were no significant difference between the radiation dose(P=0.286)in the three groups of TNC,VNC_(A),and VNC_(V)images,but there were significant difference between the total radiation dose(P<0.001)in traditional enhanced scanning mode and dual-energy scanning mode;the scores of the three-phase images of TNC,VNC_(A),and VNC_(V)were not statistically significant(P=0.701);the detection rates of the three-phase images of TNC,VNC_(A),and VNC_(V)were 82.2%,81.5%,80.8%,χ^(2)=0.091,P=0.956,showed no significant difference.Conclusion The VNC of the upper abdomen can basically replace the TNC,besides,it can reduce one phase radiation dose,which has a great advantage in returning patients.
