电针联合膈下逐瘀汤对输卵管炎性不孕大鼠TLR4、NF-κB水平的影响

摘　　要：目的:探讨电针联合膈下逐瘀汤对输卵管炎性不孕大鼠输卵管病理及TLR4、NF-κB水平的调节作用。方法:将雌性SD大鼠分为正常组、模型组、治疗组与对照组。除正常组外,其余各组大鼠进行孤养,并接受金黄色葡萄球菌输卵管注射方式造模。除正常组外各组于造模后第29 d开始给药,治疗组:电针+膈下逐瘀汤;对照组:膈下逐瘀汤;正常组:等体积蒸馏水。给药28 d后,镜下观察输卵管病理情况,观察输卵管形态及评分,免疫组化检测输卵管组织TLR4及NF-κB的表达水平。结果:与模型组比较,治疗组与对照组输卵管上皮细胞脱落减轻,炎细胞减少,胞体肿胀有不同程度改善;与模型组比较,治疗组能够明显降低TLR4及NF-κB的表达,对照组能够明显降低TLR4的表达(P<0.05);与对照组相比,治疗组在下调TLR4、NF-κB水平上作用更加明显,差异具有统计学意义。结论:电针联合膈下逐瘀汤能够改善输卵管炎性状态,抑制输卵管TLR4和NF-κB的表达,这可能是电针联合膈下逐瘀汤发挥抗炎促孕作用的机制之一;电针治疗输卵管炎性不孕作用显著,为临床推广应用提供理论支持。Objective:To investigate the effect of electro-acupuncture combined with Gexia Zhuyu Decoction on tubal pathology and the levels of TLR4 and NF-κB in rats with tubal inflammatory infertility.Methods:Female SD rats were divided into the normal group,the model group,the treatment group and the control group.Except for the normal group,the rats in the other groups were isolated and injected with Staphylococcus aureus into the fallopian tubes to establish the model.The medication was started on the 29th day after modeling,the treatment group was treated with electro-acupuncture combined with Gexia Zhuyu Decoction,the control group was treated with Gexia Zhuyu Decoction,and the model group was given equal volume of distilled water.After 28 days of intervention,the pathological changes of fallopian tubes were observed and scored under the microscope,and the expression levels of TLR4 and NF-κB were detected by immunohistochemistry.Results:Compared to those in the model group,the exfoliation of tubal epithelial cells was reduced,the inflammatory cells were decreased and the swelling of the cells was improved in the treatment group and the control group;the expressions of TLR4 and NF-κB were significantly decreased in the treatment group,and the expression of TLR4 was significantly decreased in the control group(P<0.05).The treatment group had a more significant regulating effect on the levels of TLR4 and NF-κB than the control group(P<0.05).Conclusion:Electro-acupuncture combined with Gexia Zhuyu Decoction can improve the state of salpingitis and inhibit the expressions of TLR4 and NF-κB in fallopian tubes,which may be one of the mechanisms in anti-inflammation and promoting pregnancy of the therapy.Electro-acupuncture has a significant effect on tubal inflammatory infertility,which provides theoretical support for its clinical application.
