益气温阳、活血利水法治疗慢性心力衰竭的临床观察

摘　　要：目的:观察益气温阳、活血利水法治疗慢性心力衰竭的临床疗效。方法:选取黑龙江中医药大学附属第二医院收治的心肾阳虚型慢性心力衰竭患者60例,采用随机数字表法分为对照组和治疗组各30例,对照组给予标准抗心力衰竭治疗,治疗组在西药标准治疗基础上加用具有益气温阳、活血利水作用的中药燧心胶囊,每次4粒,每日3次,连续治疗4周。观察治疗前后两组患者的中医证候积分,心功能分级,记录治疗前后左心室射血分数(LVEF)、左室短轴缩短率(FS)、舒张早期最大流速(E峰)/舒张晚期最大流速(A峰)(E/A),检测两组患者治疗前后的心型脂肪酸结合蛋白(H-FABP)、N末端脑钠肽前体(NT-proBNP)。结果:治疗前,两组患者H-FABP、NT-proBNP、LVEF、FS、E/A比较,差异无统计学意义(P>0.05),治疗4周后,治疗组中医证候疗效及心功能分级疗效均优于对照组(P<0.05),治疗组比对照组H-FABP、NT-proBNP明显降低(P<0.05),LVEF、FS、E/A明显升高(P<0.05)。结论:温阳益气、活血利水法治疗慢性心力衰竭患者心肾阳虚证具有良好的临床疗效,能有效改善患者的心功能。Objective:To observe the clinical efficacy of Yiqi Wenyang and Huoxue Lishui method in the treatment of chronic heart failure(CHF).Methods:60 patients with CHF of Yang deficiency of Heart and Kidney were randomly divided into the control group and the treatment group,with 30 cases in each group.The control group was treated with standard treatment for CHF;on which basis the treatment group was also treated with Suixin Capsule(4 capsules each time,three times a day for 4 weeks),which had the actions of Yiqi Wenyang and Huoxue Lishui.TCM syndrome score,cardiac function grade,left ventricular ejection fraction(LVEF),left ventricular fractional shortening(FS),maximum early diastolic velocity(e peak)/maximum late diastolic velocity(a peak)(E/A),heart type fatty acid binding protein(H-FABP)and n-terminal pro-brain natriuretic peptide(NT-proBNP)were observed and detected before and after the treatment in the two groups.Results:There were no statistical differences in H-FABP,NT-proBNP,LVEF,FS,E/A between the two groups before the treatment(P>0.05).After 4 weeks of the treatment,the curative effect of TCM syndromes and cardiac function grade were better in the treatment group than those in the control group(P<0.05);H-FABP and NT-proBNP were significantly lower(P<0.05),whereas LVEF,FS and E/A were significantly higher in the treatment group than those in the control group(P<0.05).Conclusion:Yiqi Wenyang and Huoxue Lishui method has a good clinical efficacy in the treatment of CHF of Yang deficiency of Heart and Kidney,it can effectively improve the heart function of the patients.
