摘　　要：目的:探讨电针内麻点对乳腺癌术后镇痛效果及疼痛介质的影响。方法:根据组间基线资料匹配原则将2017年8月—2020年8月86例行手术治疗的乳腺癌患者分为对照组43例给予常规镇痛;观察组43例术前予以电针内麻点。观察2组各时间点[针刺激前、术后2h、12h、24h和48h(T0-T4)]疼痛程度及疼痛介质水平。结果:2组T0时间点视觉模拟评分(Visual analogue scale,VAS)评分比较差异无统计学意义;观察组T1~T4时间点VAS评分均较对照组低,差异有统计学意义。2组T0时间点疼痛介质水平比较差异无统计学意义;T4时间点观察组5-羟色胺(5-hydroxytryptamine,5-HT)、去甲肾上腺素(Norepinephrine,NE)水平较对照组低,亮脑啡肽(Leucine-enkephalin,L-ENK)及β-内啡肽(β-endorphin,β-EP)水平较对照组高,差异有统计学意义。结论:电针内麻点可通过改善乳腺癌术后患者疼痛介质水平,提高术后镇痛效果。Objective:To investigate the effects of electroacupuncture on the analgesic effect and pain mediators of breast cancer surgery.Methods:According to the principle of matching baseline data between groups,86 cases of breast cancer patients who underwent surgical treatment from August 2017 to August 2020 were divided into control group 43 cases were given conventional analgesia;observation group 43 cases were given electroacupuncture and internal anesthesia before operation point.Observe the pain degree and the level of pain mediators at each time point in the 2 groups[before acupuncture stimulation,2h,12h,24h and 48h after acupuncture (T0-T4)].Results:There was no statistically significant difference in the visual analogue scale (VAS) scores between the two groups at time T0;the VAS scores of the observation group were lower than those of the control group at time T1 to T4,and the difference was statistically significant.There was no statistically significant difference in pain mediator levels between the two groups at time T0;at time T4,the observation group’s 5-hydroxytryptamine (5-HT) and norepinephrine (NE) levels were lower than those in the control group,brightening the brain The levels of Leucine-enkeph alin (L-ENK) and β-endorphin (β-EP) were higher than those of the control group,and the difference was statistically significant.Conclusion:Electroacupuncture can improve the level of pain mediators in patients after breast cancer surgery,and improve the effect of postoperative analgesia.
