带涤纶袖套的双腔导管治疗慢性关节感染的疗效观察

摘　　要：目的:探讨带涤纶袖套的双腔导管用于慢性关节感染治疗的效果。方法:收集2017年10月至2019年5月郑州大学第一附属医院骨关节科收治的关节感染患者(共计24例),其中15例为应用手术治疗及术后静脉应用抗生素的患者(对照组),9例为应用手术治疗后留置带涤纶袖套的双腔导管行局部给药的患者(试验组)。两组均行常规围手术期管理及手术处理,术后对照组经静脉应用抗生素,试验组经导管向关节腔注射抗生素。收集两组患者首次手术后复发情况、感染治愈经历的手术次数、累计应用抗生素时间等指标进行统计分析。结果:试验组感染复发率低于对照组(P=0.010),感染治愈经历的手术次数少(P=0.003),累计应用抗生素时间少(P=0.002)。结论:应用带涤纶袖套的双腔导管行关节腔注射抗生素有助于慢性关节感染的治疗。Aim:To investigate the application of double lumen catheter with polyester sleeve for the treatment of chronic joint infection.Methods:A total of 24 patients diagnosed as chronic joint infection admitted to the Department of Orthopaedic Surgery,the First Affiliated Hospital of Zhengzhou University from October 2017 to May 2019 were enrolled.The control group included 15 patients,receiving operation and intravenous antibiotics postoperatively.And the trial group included 9 patients,receiving operation and local administration of antibiotics with indwelling double lumen catheter with polyester sleeve postoperatively.Both groups received routine perioperative management after surgical procedures.The data of recurrences of infection after the first operation,times of surgeries to cure infection and the cumulative duration of antibiotic administration were collected for statistical analysis.Results:The recurrences rate of joint infection in the trial group was lower than that in the control group(P=0.010).The times of surgeries to fully eradicate joint infection in the trial group was less than that in the control group(P=0.003),and the duration of antibiotic administration was shorter than that in the control group(P=0.002).Conclusion:Intra-articular injection of antibiotics by using a double lumen catheter with polyester sleeve is beneficial for the treatment of chronic joint infections.
