摘　　要：目的:探讨阴道闭锁患者保留生育功能的手术方法。方法:对2例阴道闭锁患者采用异体脱细胞真皮(ADM)移植腹腔镜下宫颈造口及子宫阴道贯通术治疗,术后长期放置自制的宫颈支架及中空木制阴道模具,预防宫颈及阴道再粘连闭锁。结果:患者术后更换阴道模具时见生物皮片与人工阴道壁及顶端贴合良好,表面光滑;术后月经规律来潮,经血流出通畅。结论:应用ADM移植腹腔镜下宫颈造口及子宫阴道贯通术治疗阴道闭锁可保留患者生育功能,安全有效。Aim:To explore the surgical method of preserving reproductive function in patients with vaginal atresia.Methods:In two patients with vaginal atresia,acellular dermal matrix(ADM)transplantation was used for laparoscopic uterovaginal canalization.Self-made hollow wooden vaginal mold should be placed for a long time after the operation to prevent cervical and vaginal adhesion and atresia.Results:When the vaginal mold was replaced after the operation,acellular dermal matrix was observed to fit well with the artificial vaginal wall.Menstrual blood came regularly and the blood flow was unobstructed during menstruation after the operation.Conclusion:Laparoscopic ADM transplantation is a safe and effective method for the treatment of vaginal atresia with fertility retention.
