摘  要:目的比较在超低出生体质量儿中应用多种油脂肪乳(multi oil fat emulsion,SMOF)与中长链脂肪乳(medium and long-chain fat emulsion,MCT/LCT)的疗效。方法回顾性选取2019年1月—2021年3月医院超低出生体质量儿(extremely low birth weight infants,ELBW)共65例,依据肠外营养配方中的脂肪乳剂种类分为SMOF组(n=34)、MCT/LCT组(n=31),统计分析两组新生儿的临床特征、并发症发生情况、营养支持治疗及出院情况、血生化指标。结果SMOF组支气管肺发育不良发生率低于MCT/LCT组,差异有统计学意义(P<0.05)。两组新生儿的胃肠外营养相关性胆汁淤积症(parenteral nutrition associated cholestasis,PNAC)、宫外生长发育迟缓(extrauterine growth retardation,EUGR)、晚发性败血症(late onset sepsis,LOS)、新生儿坏死性小肠结肠炎(neonatal necrotizing enterocolitis,NEC)(Bell分期≥IIa)、早产儿视网膜病(retinopathy of prematurity,ROP)等发生率差异无统计学意义(P>0.05)。应用脂肪乳前后两组新生儿血生化指标对比差异均无统计学意义(P>0.05)。结论超低出生体质量儿肠外营养中应用多种油脂肪乳可降低支气管肺发育不良(bronchopulmonary dysplasia,BPD)发生率,而没有降低其他相关并发症的发生率。Objective To compare the efficacy of multi oil fat emulsion(SMOF)and medium and long-chain fat emulsion(MCT/LCT)in extremely low birth weight infants.Methods A total of 65 extremely low birth weight infants(ELBW)in our hospital from January 2019 to March 2021 were retrospectively selected.According to the types of fat emulsions in the parenteral nutrition formula,they were divided into SMOF group(n=34),MCT/LCT group(n=31),statistical analysis of the clinical characteristics,complications,nutritional support treatment and discharge status,blood biochemical indicators of the two groups of children.Results The incidence of bronchopulmonary dysplasia in the SMOF group was lower than that in the MCT/LCT group,and the difference was statistically significant(P<0.05).Parenteral nutrition associated cholestasis(PNAC),extrauterine growth retardation(EUGR),late onset sepsis(LOS),neonatal Necrotizing enterocolitis(NEC)(Bell staging≥IIa),retinopathy of prematurity(ROP)etc.,had no statistically significant difference(P>0.05).There was no significant difference in blood biochemical indexes between the two groups before and after the application of fat emulsion(P>0.05).Conclusion The application of multiple oil and fat emulsions in parenteral nutrition of extremely low birth weight infants can reduce the incidence of bronchopulmonary dysplasia(BPD),but does not reduce the incidence of other related complications.
