右美托咪定在快通道心脏麻醉诱导中对血流动力学的影响

摘　　要：目的回顾性分析在快通道心脏麻醉诱导过程中右美托咪定对其血流动力学的影响。方法进行回顾性分析,将医院自2019年6月—2020年6月200例行心脏手术的患者根据不同麻醉方案分为两组,每组100例。给予参照组常规麻醉诱导,治疗组在诱导之前应用右美托咪定,对比两组患者不同时点中MAP、HR变化以及不良反应发生情况。结果参照组与治疗组在T0、T1时点的MAP的差异无统计学意义(P>0.05),而在T2、T3时点的MAP水平中,治疗组明显低于参照组(P<0.05);参照组与治疗组在T0、T1时点的HR的差异无统计学意义(P>0.05),而在T2、T3时点的HR水平中,治疗组明显低于参照组(P<0.05);对比不良反应发生情况,治疗组发生率为6.00%,参照组发生率为5.00%,两组之间的差异无统计学意义(P>0.05)。结论在为心脏手术患者进行快通道心脏麻醉诱导之前应用右美托咪定可以稳定患者的血流动力学指标,确保循环的稳定性。Objective To retrospectively analyze the effect of dexmedetomidine on hemodynamics during induction of fast-track cardiac anesthesia.Methods Retrospective analysis,200 patients undergoing cardiac surgery in our hospital from June 2019 to June 2020 were divided into two groups according to different anesthesia schemes,with 100 cases in each group.The reference group was given routine anesthesia induction,while the treatment group was given dexmedetomidine before induction.The changes of MAP,HR and adverse reactions of the two groups were compared at different time points.Results There was no significant difference in MAP between the reference group and the treatment group at T0 and T1(P>0.05),but the MAP level of the treatment group at T2 and T3 was significantly lower than that of the reference group(P<0.05).There is no significant difference in HR between the reference group and the treatment group at T0 and T1(P>0.05),but the HR level of the treatment group at T2 and T3 is significantly lower than that of the reference group(P<0.05).Compared with the incidence of adverse reactions,the incidence of treatment group was 6.00%,while that of reference group was5.00%,and there was no significant difference between the two groups(P>0.05).Conclusion Dexmedetomidine can stabilize the hemodynamic indexes and ensure the stability of circulation before fast-track cardiac anesthesia induction for patients undergoing cardiac surgery.
