摘 要:目的:探讨左卡尼汀结合血液透析在终末期肾病(ESRD)患者中的应用价值。方法:选择2020年3月至2021年3月南昌大学第四附属医院行血液透析治疗的ESRD患者76例,按随机数字表法分为两组,各38例。对照组采用血液透析治疗,研究组加用左卡尼汀治疗,均持续治疗3个月。比较两组免疫功能、营养状况及并发症。结果:治疗前,两组成熟T淋巴细胞(CD3^(+))、辅助性T淋巴细胞(CD4^(+))、抑制性T淋巴细胞(CD8^(+))、血红蛋白(Hb)、白蛋白(Alb)、前白蛋白(PA)比较,差异无统计学意义(P>0.05);治疗后,研究组CD3^(+)、CD4^(+)、Hb、Alb、PA高于对照组,CD8^(+)低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:左卡尼汀结合血液透析能够有效改善ESRD患者免疫功能及营养状况,减少透析期间并发症的发生,值得临床应用。Objective:To explore the application value of levocarnitine combined with hemodialysis in patients with end-stage renal disease(ESRD).Methods:76 ESRD patients who underwent hemodialysis treatment in the Fourth Affiliated Hospital of Nanchang University from March 2020 to March 2021 were selected and divided into two groups according to the random number table method,with 38 cases in each group.The control group was treated with hemodialysis,and the study group was treated with levocarnitine,and the treatment was continued for 3 months.The immune function,nutritional status and complications of the two groups were compared.Results:Before treatment,the two groups of mature T lymphocytes(CD3^(+)),helper T lymphocytes(CD4^(+)),inhibitory T lymphocytes(CD8^(+)),hemoglobin(Hb),albumin(Alb)and prealbumin(PA)were compared between the two groups,the difference was not statistically significant(P>0.05);after treatment,CD3^(+),CD4^(+),Hb,Alb,PA in the study group were higher than those in the control group,and CD8^(+)was lower than the control group,the difference was statistically significant(P<0.05);the incidence of complications was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Levocarnitine combined with hemodialysis can effectively improve the immune function and nutritional status of patients with ESRD,reduce the occurrence of complications during dialysis,and is worthy of clinical application.
