膝关节隐匿性骨折低场MRI的影像表现及分析

摘　　要：目的:探讨膝关节隐匿性骨折在低场MRI中的影像表现及其诊断价值。方法:选取2015年4月~2019年5月本院收治的108例膝关节外伤患者,分析其伤后的X射线及低场MRI的影像表现。结果:108例膝关节外伤患者中,96例X射线片未见明显骨折征象,12例可疑或有间接征象提示疑似骨折,全部108例患者行低场MRI检查均为隐匿骨折阳性表现。隐匿性骨折MRI表现为骨皮质下在T1WI、T2WI上呈裂隙样、线样低信号带,T2WI序列上低信号周围可见边界模糊的斑片状高信号,STIR序列显示更加清楚。结论:MRI能够凭借本身优秀的组织分辨力,清楚显示隐匿性骨折,尤其是X射线检查阴性而临床表现与体征怀疑隐匿性骨折时,MRI则是最佳影像学检查方法。Objective:To investigate the low field MRI features of occult fracture of the knee and its diagnostic value.Methods:108 cases of knee trauma patients admitted to our hospital from April 2015 to May 2019 were selected,and their post-injury X-ray and low-field MRI imaging manifestations were analyzed.Results:Among 108 cases of knee joint trauma,96 cases had no obvious sign of fracture on X-ray film,12 cases had suspicious or indirect sign,all 108 cases had positive sign of occult fracture on low field MRI.The MRI findings of occult fracture showed cleft-like and linear-like hypointense bands on T_(1)WI and T_(2)WI subcortical areas,and patchy high signal around hypointense on T_(2)WI sequence,especially on STIR sequence.Conclusion:MRI is the best imaging inspection method to clearly show occult fracture,especially when X-ray examination is negative and clinical manifestations and signs suspect occult fracture.
