摘 要:目的:应用心血管整体评分评价电子胎心监护异常的胎儿中心功能不全,以及预测不良妊娠结局的价值。方法:选取2016年1月~2020年9月胎心监护异常的胎儿465例,同时进行胎儿生物物理评分与胎儿心血管整体评分,分别评价二者在不良妊娠结局中的差异,比较二者的ROC曲线。结果:在胎心监护异常的晚孕胎儿中,不良妊娠的发生率6.06%,CVPS分值在不良妊娠组与结局良好组分别为(7.67±1.34)、(9.77±1.00),两组间差异有显著性意义。不良妊娠结局组的胎儿BPS分值(4.86±1.42),妊娠结局良好的胎儿BPS为(6.67±1.50),BPS在不良妊娠组与结局良好组间差异无统计学意义。胎儿生物物理评分与胎儿心血管整体评分比较,二者的ROC曲线下面积分别是0.875、0.957,ROC曲线二者比较差异有统计学意义。结论:胎儿心功能评价是监护胎儿宫内安危的重要组成部分,CVPS全面、简便的参数系统为产科干预及预后提供了更确切、更客观的依据。Objective:To evaluate the value of cardiovascular profile score in the fetal central dysfunction with abnormal electronic fetal monitoring and predicting adverse pregnancy outcomes.Methods:A total of 465 fetuses with abnormal fetal monitoring from January 2016 to September 2020 were selected.The fetal biophysical score and the fetal cardiovascular profile score were performed to evaluate the difference about the adverse pregnancy,and the ROC curve was compared.Results:In the third trimester with abnormal fetal monitoring,the incidence of adverse pregnancy was 6.06%.The CVPS scores in the adverse pregnancy group and the good outcome group were(7.67±1.34)and(9.77±1.00),respectively.The BPS score of the adverse pregnancy outcome group was(4.86±1.42),and that of the good pregnancy outcome group was(6.67±1.50).There was no significant difference between the two groups.The area under the ROC curve of the two groups was 0.875 and 0.957,the difference of ROC curve between the two is statistically significant.Conclusion:The evaluation of fetal heart function is an important part for clinical close monitoring of the fetal intrauterine safety.The comprehensive and simple parameter system of CVPS provides more accurate and objective basis for obstetric intervention and prognosis.
